摘要
目的研究胃镜引导下置入十二指肠营养管治疗食管癌术后并发吻合口瘘的疗效。方法选择120例食管癌术后并发吻合口瘘患者,随机分为2组。对照组采取常规的静脉营养支持以及胃肠减压治疗,观察组在胃镜的引导下置入十二指肠营养管治疗。比较2组的临床治愈率、瘘口愈合时间、住院总费用以及住院时间;并比较2组的前白蛋白、白蛋白、血红蛋白、转铁蛋白等营养状态指标。结果观察组的临床治愈率明显高于对照组(P<0.05),且观察组的瘘口愈合时间、住院总费用以及住院时间均明显低于对照组(P<0.05)。观察组的前白蛋白、白蛋白、血红蛋白、转铁蛋白水平均明显高于对照组(P<0.05)。2组治疗后的SF-36量表评分值均明显升高(P<0.05),且观察组明显高于对照组(P<0.05)。结论对于食管癌术后并发吻合口瘘患者而言,胃镜引导下置入十二指肠营养管是一种方便、安全以及有效的肠内营养方法,在其营养治疗中发挥着重要的支持效果,值得临床应用推广。
Objective To investigate the effect of duodenal nutrition tube guided by gastroscopy on anastomotic leakage after esophagus cancer operation.Methods Selected 120 cases of patients with anastomotic leakage after esophagus cancer operation were divided into 2 groups randomly.The control group was given routine intravenous nutrition support and gastrointestinal decompression,while the observation group was given Duodenal Nutrition Tube under the guidance of gastroscope.The clinical cure rate,fistula healing time,total hospitalization expenses and length of stay were compared between the 2 groups.Results The clinical cure rate of the observation group was significantly higher than that of the control group(P<0.05),and the fistula healing time,total hospitalization expenses and hospitalization time of the observation group were significantly lower than those of the control group(P<0.05);The levels of prealbumin,albumin,hemoglobin and transferrin in the observation group were significantly higher than those of the control group(P<0.05).After treatment,the scores of SF-36 scale in both groups were significantly increased(P<0.05),and the observation group was significantly higher than the control group(P<0.05).Conclusion For esophageal cancerpatients with anastomotic fistula after surgery,gastroscopy-guided duodenal nutrition tube placement is a convenient,safe and effective method of enteral nutrition,in its nutritional treatment plays an important supporting effect.
