摘要
糖尿病肾病是糖尿病常见的并发症,但目前缺乏有效治疗手段。肠道菌群通过影响血流动力、糖脂代谢、内毒素积累和炎症等方面影响糖尿病肾病发病与结局。查阅相关文献,对运用中医中药、益生菌、益生元等手段调节肠道菌群干预糖尿病肾病的效果和相关机制进行综述。发现调节肠道菌群的各种新疗法确实有效,但需要更多高质量随机对照大样本的临床试验来界定其作用,而中医药的多靶点整体治疗可以弥补其他疗法单一性干预的不足,未来可以探索联合中医内外治法及益生元益生菌,辅助或者代替西药治疗糖尿病肾病。
Diabetic nephropathy(DN) is a common complication of diabetes,which lacks of effective treatment.Intestinal flora affects the incidence and outcome of DN by affecting blood flow dynamics,metabolisms of glucose and lipid,endotoxin accumulation and inflammation formation.This study reviewed the effects and related mechanisms of intestinal flora regulation on DN by using TCM medication,as well as prebiotics and probiotics therapies.It is found that various new therapies for regulating intestinal flora are effective,but more high-quality randomized controlled clinical trials with large samples are needed to define their effects.The multi-target holistic therapy of TCM can make up for the shortcomings of single intervention of other therapies.In the future,the combination of TCM internal and external therapies and prebiotics and probiotics can be explored to assist or replace western medication in the treatment of DN.
