摘要
目的研究固肾健脾化湿方结合坎地沙坦酯对糖尿病肾病的治疗效果。方法将94例糖尿病肾病分为对照组和观察组各47例,对照组采用口服坎地沙坦酯片治疗。观察组在对照组治疗基础上加用固肾健脾化湿方对糖尿病肾病患者进行治疗。14 d为1个疗程。共治疗6个疗程。对比两组治疗前后治疗疗效、治疗前后24 h蛋白尿、24 h尿微蛋白、β2微球蛋白水平、中医症候积分、血肌酐、尿素氮水平、炎症因子IL-6、IL-18及TNF-α水平。结果研究结果显示,观察组总有效率为70.21%(33/47),对照组总有效率为44.68%(21/47),观察组总有效率明显高于对照组,差异具有统计学意义(P<0.05)。观察组24 h蛋白尿、24 h尿微蛋白、β2微球蛋白水平对比均低于对照组,差异具有统计学意义(P<0.05)。观察组面部水肿、神疲乏力、肢体麻痛低于对照组,差异具有统计学意义(P<0.05)。观察组血肌酐、尿素氮对比均低于对照组,差异具有统计学意义(P<0.05)。观察组IL-6、IL-18及TNF-α对比显著低于对照组(P<0.05)。结论固肾健脾化湿方结合坎地沙坦酯治疗糖尿病肾病疗效突出,与单纯西医治疗相比,充分体现了中西医结合治疗糖尿病肾病的优势,通过减轻了患者炎症因子的水平,减少了尿白蛋白指标,延缓了肾损害的发展进程,改善了临床症状,值得临床推广运用。
Objective To study the therapeutic effect of Gushen Jianpi Huashi Formula combined with Candesartan Cilexetil on diabetic nephropathy.Methods Ninety-four patients with diabetic nephropathy admitted to our hospital from January 2018 to March 2019 were divided into control group and observation group according to random number table method:47 patients in control group and 47 patients in observation group.The control group was treated with Candesartan CilexetilTablets orally.The observation group was treated with Gushen Jianpi Huashi Formula on the basistreatment of the control group.Fourteen days was a course of treatment.Two groupswere treated for 6 courses.The therapeutic effects of two groups were compared.The levels of proteinuria,urinary microprotein andβ-2 microglobulin before and after treatment were compared.The scores of TCM symptoms,serum creatinine and urea nitrogen levels before and after treatment were compared.The levels of inflammatory factors IL-6,IL-18 and TNF-αbefore and after treatment were compared between the two groups.Results The results showed that the total effective rate of the observation group was 70.21%(33/47),and that of the control group was 44.68%(21/47).The total effective rate of the observation group was significantly higher than that of the control group(P<0.05).The levels of 24-hour proteinuria,24-hour urinary microprotein and beta-2 microglobulin in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05).Facial edema,mental fatigue and limb pain in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05).The serum levels of creatinine and urea nitrogen in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05).The levels of IL-6,IL-18 and TNF-αin the observation group were significantly lower than those in the control group(P<0.05).Conclusion In conclusion,Gushen Jianpi Huashi Form
