摘要
目的分期外周血嗜酸粒细胞计数(EOS)评估慢性鼻-鼻窦炎治疗效果的价值。方法于2017年12月1日-2018年12月1日纳入作者医院慢性鼻-鼻窦炎患者82例,均行手术治疗,根据术后病情分为控制组、未控制组,对比两组患者临床资料(性别、年龄、伴发病情、病程、手术史、CT评分、内镜评分、VAS评分以及外周血单核细胞、淋巴细胞、中性粒细胞、EOS、白细胞计数、IL-33)。对比糖皮质激素治疗后不同疗效组患者VAS评分以及外周血EOS、白细胞计数、IL-33。分析外周血EOS计数与各指标的关系。结果(1)病情控制组患者伴有病情与对照组差异显著,且该组VAS分值、外周血IL-33水平及EOS计数较未控制组低。(2)糖皮质激素治疗显效组、有效组、无效组VAS分值、外周血IL-33水平、EOS计数差异均有统计学意义。(3)Pearson分析确定EOS与IL-33呈正相关(r=0.247,P=0.0487),与VAS分值呈正相关(r=0.615,P=0.0000)。结论外周血嗜酸粒细胞计数可用于慢性鼻-鼻窦炎疗效评估中,与病情指标相关,可联合应用辅助临床疗效评估。
Objective To evaluate the efficacy of(EOS) in the treatment of chronic rhinosinusitis by staging peripheral blood eosinophil count. Methods From December 1, 2017 to December 1, 2018, 82 patients with chronic rhinosinusitis in our hospital were treated by operation. They were divided into the control group and the uncontrolled group according to their postoperative conditions. The clinical data(sex, age, age)of the two groups were compared. CT score, endoscopy score, VAS score and peripheral blood monocyte,lymphocyte, neutrophil, EOS, leukocyte count, IL-33)were associated with the disease, course of disease, history of operation, CT score,endoscopy score, and VAS score. Comparison of VAS score, EOS, leukocyte count and IL-33. in patients with different therapeutic effects after glucocorticoid treatment The relationship between EOS count in peripheral blood and each index was analyzed. Results(1)There were significant differences between the two groups in the condition control group and the condition control group, and the VAS score,IL-33 level in peripheral blood and EOS count in the group were lower than that of the uncontrolled group.(2) There were significant differences in VAS score, IL-33 level in peripheral blood and EOS count among the performance group, the effective group and the ineffective group.(3)Pearson analysis confirmed that EOS was positively correlated with IL-33(r = 0.247, P = 0.0487)and VAS score(r =0.615, P = 0.0000). Conclusion The eosinophil count in peripheral blood can be used to evaluate the curative effect of chronic rhinosinusitis.
