摘要
目的探讨髋关节置换术男性患者在腰麻后行侧卧位留置尿管的临床效果。方法选取茂名市人民医院麻醉手术科100例采取椎管内麻醉实施髋关节置换术的男性患者,排除尿道畸形和前列腺增生者。按照住院号尾数单双号分为观察组和对照组,每组各50例。观察组在麻醉诱导成功后行侧卧位行导尿术,对照组在麻醉成功后改平卧位留置尿管,再调整为手术体位。采用卡方检验和t检验分析及比较两组间的尿道损伤、一次导尿成功率、手术准备时间和单侧麻醉效果。结果观察组和对照组分别有3例(6%)和4例(8%)尿道损伤,差异无统计学意义(P>0.05);两组的一次导尿成功率比较,差异也无统计学意义(P>0.05);手术准备时间(t=20.802)和单侧麻醉效果(Х^2=59.278)相比观察组均优于对照组,且差异具有统计学意义(P<0.01)。结论髋关节置换术男性患者麻醉后采取侧卧位导尿,有利于缩短手术准备时间和提升单侧麻醉效果,且不增加尿道损伤。
Objective To investigate the effect of lateral decubitus catheterization after lumbar anesthesia in male patients undergoing hip replacement.Methods A total of 100 male patients undergoing hip replacement under intraconical anesthesia were included in this study,the patients with urethral deformity or prostatic hyperplasia were excluded.According to the admission number,the patients were divided into the observation group and the control group with 50 cases in each group by even and odd numbers.The observation group underwent lateral decubitus catheterization after successful induction of anesthesia,and the control group were changed to supine position for indwelling urinary catheter.The urethral injury,success rate of primary urethral catheterization,operation preparation time and unilateral anesthesia effect were compared between the two groups.Results There were three cases(6%)and four cases(8%)of urethral injury in the observation group and the control group,respectively,with no significant difference(P>0.05).There was also no significant difference in the success rate of one-time catheterization between the two groups(P>0.05).The preparation time of operation(t=20.802)and the effect of unilateral anesthesia(Х^2=59.278)in the observation group were better than those in the control group,and the difference was statistically significant(both P<0.01).Conclusion Lateral decubitus urethral catheterization after anesthesia for male patients undergoing hip replacement is beneficial to shorten the operation preparation time and improve the effect of anesthesia without increasing urethral injury.
