摘要
目的探讨临床药师参与药物治疗方案的制定与药学监护,使用药物更加安全、有效。方法临床药师参与1例丙戊酸引起中性粒细胞减少的药物治疗过程,分析该患者中性粒细胞减少的可能原因、发生机制、以及对此的预防措施,从药学的思维去思考临床实际问题,为临床用药提供建设性意见,适时进行患者用药教育等药学监护。结果临床药师的用药建议被医生采纳,用药教育被患者接受,药品不良反应也得到及时发现和控制。结论临床药师应重视药物不良反应的预防和治疗,并利用其自身专业优势,以药物可能对血液系统所造成的影响为切入点,结合患者的既往用药史、病情等具体情况作出综合分析,协助医师制订和优化药物治疗方案,保证了用药的安全、有效。
OBJECTIVE To explore the participation of clinical pharmacists in the formulation of drug treatment programs and pharmaceutical care,so as to promote the rational use of drugs in clinical practice. METHODS Clinical pharmacists participated in the treatment of a patient with neutropenia induced by valproic acid.The possible causes,mechanism and preventive measures of neutropenia were analyzed. The practical clinical problems were considered from the perspective of pharmacy,and constructive suggestions were provided for the clinical use of drugs. Pharmaceutical care such as drug education. RESULTS Clinical pharmacists' drug recommendations were accepted by doctors,drug education was accepted by patients,and adverse drug reactions were detected and controlled in time. CONCLUSION Clinical pharmacists should pay attention to the prevention and treatment of adverse drug reactions,make use of their own professional advantages,and assist doctors to formulate and optimize the medication program to ensure the safety and effectiveness use of medication.
