摘要
目的探讨甲状腺转移癌的临床病理学特点,以提高对甲状腺转移癌的认识。方法回顾性分析青岛大学附属医院2010年1月—2018年6月诊断的14例甲状腺转移癌的临床特点以及病理学表现。结果2010年1月—2018年6月期间共诊断甲状腺恶性肿瘤11085例,其中转移性恶性肿瘤14例(均为转移癌),占甲状腺恶性肿瘤的0.14%。结合临床病史、病理形态学和免疫组织化学检查,确定14例转移癌的原发灶分别为肺腺癌5例、肺小细胞癌4例、乳癌3例、食管鳞状细胞癌2例。13例出现颈部包块,1例查体偶然发现甲状腺结节。有3例病人首发症状为甲状腺结节,穿刺活检病理均诊断为肺腺癌转移。结论甲状腺转移癌临床罕见,确诊需要结合临床病史、病理形态学和免疫组织化学检查。肺癌是甲状腺转移癌最常见的原发灶。临床症状以颈部包块为主,甲状腺结节常为肺腺癌所致甲状腺转移癌的首发症状。
Objective To investigate the clinicopathological characteristics of thyroid metastatic carcinoma, and to improve the understanding of thyroid metastatic carcinoma. Methods The clinical features and pathological findings of 14 patients diagnosed with thyroid metastatic carcinoma at the Affiliated Hospital of Qingdao University from January 2010 to June 2018 were analyzed retrospectively. Results From January 2010 to June 2018, a total of 11 085 cases of thyroid malignant tumors was diagnosed , including 14 cases of metastatic malignant tumors (all metastatic cancers), accounting for 0.14% of thyroid malignant tumors. With reference to clinical history, pathomorphology, and immunohistochemical examination, the primary tumors of the 14 cases of metastatic carcinoma were lung adenocarcinoma in 5 cases, small cell lung cancer in 4 cases, breast cancer in 3 cases, and esophageal squamous cell carcinoma in 2 cases. Thirteen cases had a neck mass, and one case was accidentally found to have a thyroid nodule on physical examination. Three patients had the first symptom of thyroid metastasis, and all were diagnosed with lung adenocarcinoma metastasis by needle biopsy. Conclusion Thyroid metastatic carcinoma is rare in clinical practice, and its diagnosis needs to be combined with clinical history, pathomorphology, and immunohistochemistry. Lung cancer is the most common primary lesion of thyroid metastasis. Cervical mass is the main clinical symptom. Thyroid metastasis is often the first symptom of thyroid metastatic carcinoma caused by lung adenocarcinoma.
