摘要
目的:探讨单、双相抑郁障碍患者的失眠程度和日间嗜睡程度变化的特点,为临床单、双相抑郁障碍的鉴别诊断提供新的思路。方法:对符合《美国精神障碍诊断与统计手册(第5版)》单、双相抑郁障碍诊断标准的深圳市康宁医院门诊就诊的54例单相抑郁患者,37例双相抑郁患者和40例健康对照者,采用失眠严重程度指数(insomnia severity index,ISI),Epworth嗜睡量表(Epworth sleeping scale,ESS)进行评分及分析。结果:单相抑郁组、双相抑郁组ISI和ESS评分高于对照组,差异有统计学意义(P<0.01),单相抑郁组ISI评分比双相抑郁组高,差异有统计学意义(P<0.01),单相抑郁组和双相抑郁组的ESS评分差异无统计学意义(P>0.05)。结论:单、双相抑郁障碍患者的失眠状况均较严重,单相抑郁障碍的失眠程度较双相抑郁更加严重,日间嗜睡症状在单、双相抑郁障碍中均较严重,临床诊疗过程中应加以重视。
Objective: To compare the insomnia and hypersomnia in patients with unipolar or bipolar disorder,and to find out whether there are differences between the two diseases,so as to provide new ideas for the differential diagnosis. Methods: To recruit outpatients who were treated in Shenzhen Kangning Hospital, the diagnosis is based on the DSM- 5. Fifty- four patients with unipolar depression, thirty- seven patients with bipolar depression, and forty health controls were recruited. Their general demographic information was collected. Insomnia severity index(ISI) and Epworth sleep scale(ESS) were evaluated for three groups respectively under the guidance of clinical research physicians and then analyzed. Results:Compared with control group,the scores of ISI and ESS of the patients with unipolar depression and the patients with bipolar depression were significantly higher;the difference was statistically significant(P<0.01). The insomnia severity of patients with unipolar depression was higher than the patients with bipolar depression(P<0.01). There was no difference between the scores of ESS of patients with unipolar depression and bipolar depression (P>0.05). Conclusion: Insomnia and hypersomnia were common in patients with unipolar depression or bipolar depression. Unipolar depression was worse than bipolar depression in terms of insomnia. We should pay more attention to hypersomnia of patients with unipolar or bipolar depression.
