摘要
目的探讨有创-无创序贯通气在心脏术后患者中的应用。方法选取2017年10月至2018年10月我院心胸外科收治的行心脏手术后气管插管有创呼吸机辅助呼吸的84例患者为研究对象,随机等分为对照组和试验组,对照组接受有创机械通气,撤机后给予鼻导管吸氧;试验组接受有有创-无创序贯通气,撤机后给予鼻导管吸氧,比较两组机械通气的时间、一次性撤机成功情况、呼吸机相关性肺炎的发生情况。结果试验组机械通气时间、一次性撤机成功的情况、呼吸机相关性肺炎发生情况均优于对照组,差异有统计学意义(P<0.05)。结论及时对心脏术后的患者进行评估实施有创-无创序贯通气,有利于术后恢复,缩短机械通气的时间呼吸机时间,提高一次性撤机情况,值得临床推广应用。
Objective To investigate the application of iinvasive-noninvasive consequent ventilation in patients after cardiac surgery.Methods To select 84 cases of patients with invasive ventilator-assisted breathing for endotracheal intubation after the cardiac surgery who were treated in the cardiothoracic surgery department of our hospital from October 2017 to October 2018 as the study subjects.They were divided into control group and experimental group with the same number of patients randomly.The control group was given invasive ventilation and after the ventilation was removed,these patients were given nasal catheter for oxygen.The experimental group was given invasive-noninvasive consequent ventilation,and after the ventilation was removed,these patients were given nasal catheter for oxygen.The time of mechanical ventilation,the success of the one-time weaning and the incidence of ventilator-associated pneumonia in the two groups were compared.Results The time of mechanical ventilation,the success of the one-time successfully weaning and the incidence of ventilator-associated pneumonia in the experimental group were better than those in the control group,and the difference was statistically significant(P<0.05).Conclusion If we can evaluate the patients after cardiac surgery in time and give them invasive-noninvasive consequent ventilation,we can help them to recover after the surgery.Meanwhile,it can shorten the time of mechanical ventilation and the time of ventilator,and improve the condition of one-time extubating,which was worthy of clinical promotion and application.
