摘要
目的 探讨重组人脑利钠肽联合硝酸甘油对慢性心力衰竭患者心室重构及心功能的影响。方法 将2016年5月至2018年2月收治的98例慢性心力衰竭患者,按随机双盲法分为对照组49例及观察组49例。对照组给予硝酸甘油治疗,观察组在此基础上给予重组人脑利钠肽治疗。比较两组患者心室重构指标及心功能指标。结果 治疗后4周,两组左室舒张末期内径(LVEDD)、左室间隔厚度(IVST)及左室质量指数(LVMI)均升高(P<0.05),但观察组上述指标低于对照组,差异有统计学意义(P<0.01);治疗后4周,两组左室内径缩短率(LVFS)、左室射血分数(LVEF)、左室舒张时间(IVRT)均升高(P<0.05),且观察组高于对照组,差异有统计学意义(P<0.01)。结论 重组人脑利钠肽联合硝酸甘油治疗慢性心力衰竭,能够有效抑制心室重构,改善患者心脏舒缩功能,值得临床推广。
Objective To investigate the effect of recombinant human brain natriuretic peptide combined with nitroglycerin on ventricular remodeling and cardiac function in patients with chronic heart failure. Methods 98 patients with chronic heart failure admitted to hospital from May 2016 to February 2018 were divided into control group(49 cases) and observation group(49 cases) according to random and double-blindness method.The control group were treated with nitroglycerin,while based on which the observation group were treated with recombinant human brain natriuretic peptide.And then,the indexes of ventricular remodeling and cardiac function were compared between the two groups. Results Four weeks after treatment,left ventricular end-diastolic diameter(LVEDD),left ventricular septal thickness(IVST) and left ventricular mass index(LVMI) increased in both groups( P <0.05),but the above indexes in the observation group were lower than those in the control group,difference was statistically significant( P <0.01).Four weeks after treatment,left ventricular fraction shortening(LVFS),left ventricular ejection fraction(LVEF) and left ventricular diastolic time(IVRT) increased in both groups( P <0.05),and the above indexes in the observation group were higher than those in the control group,difference was statistically significant( P <0.01). Conclusion Recombinant human brain natriuretic peptide combined with nitroglycerin can effectively inhibit ventricular remodeling,and improve cardiac systolic and diastolic function in patients with chronic heart failure,which is worthy of clinical promotion.
