摘要
糖尿病足感染是糖尿病足溃疡难愈合、截肢及死亡的重要原因。把握糖尿病足感染分级及临床特点,多学科协作,总结糖尿病足感染的发生、发展、变化的规律,建立分期辨证、内外合治糖尿病足感染的中西医结合综合治疗方案。根据糖尿病足感染的特点,分急性进展期、缓解期、稳定期三期及四级三度论治,注重适时引流及清除坏死组织,重视整体治疗与局部治疗相结合,注重护场理论,重视温病学说的应用,截断、扭转病势的发展,注重祛瘀生新、煨脓湿润、箍围疗法的临床应用。在保持引流通畅,邪有出路的基础上,中医综合治疗的序贯方案,能加速感染的控制,减少抗生素的使用强度和缩短抗生素的疗程,降低抗生素的副作用,促进坏死组织脱落及加速疮面愈合,降低截肢率和死亡率,提高生命质量。
Diabetic foot infection was an important cause of difficult healing,amputation and death of diabetic foot ulcers.Integrated traditional Chinese and western medicine therapy of internal and external treatment differentiation on stages in treatment of diabetic foot infection was established by summarizing the occurrence,development and change of diabetic foot infection,grasping its infection grade and clinical characteristics,multi-disciplinary and multidisciplinary collaboration.Diabetic foot infection were divided into acute progressive stage,remission stage,stable stage and four grades three degree according to its clinical characteristics.During treatment,timely drainage and removal of necrotic tissue,the combination of holistic therapy and local treatment,Hu-Chang theory and theory of epidemic febrile disease was paid attention to cut off the development of torsional disease and the clinical application of removing blood stasis,Weinong Shirun and the therapy of Guwei were emphasized on.On the basis of keeping the circulation open and evil out of the way,the sequential treatment of TCM comprehensive treatment could speed up the control of infection,reduce the use of antibiotics,shorten the course of antibiotics,reduce the side effects of antibiotics,promote the exfliation of necrotic tissue and accelerate wound healing,reduce the rate of amputation rate and mortality and improve the quality of life.
