摘要
目的分析比较腮腺浅叶部分切除术(PSP)与腮腺浅叶切除术(SP)治疗腮腺浅叶良性肿瘤的效果。方法选取我院84例腮腺浅叶良性肿瘤患者为研究对象,采用随机数字表法分为行PSP术组(观察组,n=42)和行SP术组(对照组,n=42)。记录两组手术相关指标(术中失血量、手术时间、解剖面神经分支数、切口长度)及术后并发症[味觉出汗综合征(Frey综合征)、暂时性面瘫、涎瘘、耳垂麻木]发生情况差异,并比较两组术前及术后6个月时外貌负性情绪[体像担忧问卷(BICI)]、身体意象状态[身体意象量表(BIS)]差异。结果观察组术中失血量、手术时间、解剖面神经分支数、切口长度及术后并发症总发生率均低于对照组(P<0.05)。术后6个月时,两组BICI、BIS评分均较术前升高(P<0.05),且对照组高于观察组(P<0.05)。结论 PSP手术对腮腺浅叶良性肿瘤患者治疗效果优于SP术式,可优化手术相关指标,且降低术后并发症发生风险,还能减轻患者外貌负性情绪,改善其身体意象状态,于提高患者生活质量有积极意义。
Objective To compare the effects of partial superficial parotidectomy(PSP)and superficial parotidectomy(SP)on benign tumor of parotid gland in superficial lobe.Methods A total of 84 patients with benign tumors of parotid gland in superficial lobe in our hospital were selected as the research subjects,and they were randomly divided into PSP group(observation group,n=42)and SP group(control group,n=42).The indicators related to operation(intraoperative blood loss,operation time,branch numbers of facial nerves and length of incision)and postoperative complications[gustatory sweating syndrome(Frey syndrome),temporary facial paralysis,salivary fistula,earlobe numbness]in the two groups were recorded,and the negative emotions of appearance[Body image concerns questionnaire(BICI)]and states of body image[Body image scale(BIS)]in the two groups were compared before and 6 months after operation.Results The intraoperative blood loss,operation time,branchnumbers of facial nerves,length of incision and incidence of postoperative complications in the observation group were all lower than those in the control group(P<0.05).6 months after operation,the scores of BICI and BIS in the two groups were all higher than those before operation(P<0.05),and the scores in the control group were higher than those in the observation group(P<0.05).Conclusion Compared with SP,PSP,whose effect is better for patients with benign tumor of parotid gland in superficial lobe,can optimize the indicators related to operation,reduce the risks of postoperative complications,alleviate the negative emotions of appearance,improve their states of body image,and it also has positive significance to improve patients'quality of life.
