摘要
目的探讨Elabela在子痫前期胎盘中的表达情况及其意义。方法在武警后勤学院附属医院产科收集2016年12月至2018年3月子痫前期产妇34例(子痫前期组)及健康产妇40名(对照组),检测胎盘中Elabela与APJ受体mRNA相对表达水平和蛋白表达情况,细胞外信号调节激酶1/2(ERK1/2)磷酸化水平,并对Elabela、APJ受体、活化的ERK1/2与血压的关系进行相关性分析。结果与对照组比较,子痫前期组胎盘中Elabela和APJ受体mRNA相对表达水平降低(P<0.05);免疫组化结果显示Elabela和APJ受体在胎盘中的表达部位基本一致,主要表达于滋养层细胞和蜕膜细胞,散在表达于血管内皮细胞,主要表达部位为细胞浆和细胞膜;相比于对照组,Elabela在子痫前期组中蛋白表达降低(12.13±1.93比15.95±2.08,P=0.002),APJ受体在子痫前期组的蛋白表达也是降低的(9.22±1.56比14.57±4.17,P=0.009);两组之间ERK1/2总蛋白表达水平差异无统计学意义(对照组1.18±0.18比子痫前期组0.85±0.10,P=0.09),而相比于对照组,子痫前期组中ERK1/2活化水平明显升高(1.10±0.17比0.47±0.11,P=0.03)。Pearson相关分析结果显示Elabela mRNA水平与收缩压、舒张压呈负相关(r=-0.46,P=0.01;r=-0.44,P=0.01);APJ受体mRNA水平与收缩压、舒张压呈负相关(r=-0.47,P=0.006;r=-0.45,P=0.01);ERK1/2活化水平与收缩压呈正相关(r=0.45,P=0.04),与舒张压相关性无统计学意义(r=0.31,P=0.17)。结论 Elabela可能在胎盘发育、血管形成方面起着积极的作用,它的表达下调引起滋养层细胞浸润不足和随后的螺旋动脉重塑障碍,可能是子痫前期发病的因素之一。
Objective To investigate the expression of Elabela in preeclamptic placenta. Methods Between December 2016 and March 2018, 34 cases of preeclampsia parturients(preeclampsia group) and 40 cases of healthy parturients(control group) were collected from Obstetrics Department of Affiliated Hospital, Armed Police Logistics University. The mRNA and protein expression of Elabela and APJ receptor in placenta as well as the phosphorylation of extracellular signal-regulated kinase 1/2(ERK1/2) were detected. The relationship between Elabela, APJ receptor, activated ERK1/2 and blood pressure was analyzed. Results Compared with the control group, the mRNA levels of Elabela and APJ receptor were decreased in the preeclamptic placenta(P<0.05). The expression sites of Elabela and APJ receptors in the placenta were basically the same, mainly expressed in the cytoplasm and membrane of trophoblast cells, decidual cells, and scatteredly in vascular endothelial cells, as indicated by immunohistochemistry. The protein levels of Elabela and APJ receptor were significantly lower in the preeclampsia group than those in the control group(Elabela: 12.13±1.93 vs 15.95±2.08, P=0.002;9.22±1.56 vs 14.57±4.17, P=0.009). The protein level of total ERK1/2 had no statistical difference between the two groups(1.18±0.18 vs 0.85±0.10, P>0.05), but the phosphorylated ERK1/2 was significantly higher in the preeclampsia group compared with the control group(1.10±0.17 vs 0.47±0.11, P<0.05). Pearson correlation analysis showed that Elabela mRNA negatively correlated with systolic blood pressure(r=-0.46, P=0.01) and diastolic blood pressure(r=-0.44, P=0.01), and APJ receptor mRNA was inversely related to systolic blood pressure(r=-0.47, P=0.006) and diastolic blood pressure(r=-0.45, P=0.01). There was a positive correlation between activated ERK1/2 and systolic blood pressure(r=0.45, P=0.04), whereas no correlation with diastolic blood pressure(r=0.31, P=0.17). Conclusion Elabela may play an active role in placental development and angiogenesis. It
