摘要
目的探讨妊娠期间高血压(HIP)对女性产后动脉硬化的影响。方法 1976年10月至2016年12月期间分娩且分娩资料完整的开滦女性职工共5266例,在2010年3月至2017年8月对该人群进行臂踝动脉脉搏波传导速度(baPWV)测定,排除资料不完整者,最终纳入2709例进行统计分析。将研究对象分为HIP和非HIP组,比较组间资料的差异;采用多因素Logistic回归分析HIP对baPWV的影响。结果 2709例研究对象中HIP组330例,非HIP组2379例,且HIP组的baPWV较非HIP组高[(13.49±2.64)比(12.48±2.37)m/s,P<0.05]。校正混杂因素后,多因素Logistic回归分析显示,HIP是baPWV≥14 m/s的危险因素,OR(95%CI)为1.45(1.06~1.98)。对不同亚组人群的分析显示,在非糖尿病、年龄<50岁及分娩后时间<10年组中HIP是baPWV≥14 m/s的危险因素[OR(95%CI)分别为1.49(1.08~2.07),1.77(1.24~2.51),2.14(1.07~4.28),均P<0.05]。结论HIP对女性产后动脉硬化有影响,且尤以在非糖尿病、年龄<50岁及分娩后<10年的人群中为著。
Objective To investigate the effect of hypertension during pregnancy on postpartum arteriosclerosis in women. Methods A total of 5266 female workers in Kailuan Group who gave childbirth during October 1976 to December 2016 and with complete delivery data were recruited. From March 2010 to August 2017, brachial-ankle pulse wave velocity(baPWV) was measured in this population. Finally, 2709 participants were included for statistical analysis. The participants were divided into hypertension in pregnancy(HIP) and non-HIP groups, and the differences between groups were compared. The effect of HIP on baPWV was analyzed by multivariate Logistic regression analysis. Results In 2709 subjects, 330 were HIP and 2379 were non-HIP. BaPWV was higher in HIP group than that in non-HIP group [(13.49±2.64) vs(12.48±2.37) m/s, P<0.05]. After adjusting for the confounder factors, multivariate Logistic regression analysis showed that HIP was a risk factor for baPWV≥14 m/s and OR(95% CI) was 1.45(1.06-1.98). The analysis of different subgroups showed that HIP was a risk factor for baPWV>14 m/s in subjects with non-diabetes, age<50 years old and post-delivery<10 years[OR(95% CI) was 1.49(1.08-2.07), 1.77(1.24-2.51), 2.14(1.07-4.28) respectively, all P<0.05]. Conclusion HIP has an effect on postpartum atherosclerosis in women, especially in subjects with non-diabetic, age<50 years old and post-delivery<10 years.
