摘要
目的分析M-Tang法肌腱缝合技术修复手指Ⅱ区屈肌腱的临床疗效。方法自2013年11月至2016年12月收治77例(96指)Ⅱ区指深屈肌腱断裂患者,应用M-Tang法肌腱缝合技术修复,术后早期进行保护性主、被动功能锻炼,随访并测量患指的关节主动活动度,应用Strickland和Glogovac标准进行患指功能评定。对患者的年龄、性别、A2滑车切开与否、伴随损伤情况、随访时间长短及伤后进行手术时间等影响因素进行统计学分析。结果失访19例,58例(70指)获得随访,时间为4~27个月,平均16个月。70指中优43指,良13指,中10指,差4指,优良率为80%.没有发生肌腱再断裂的手指。患者中年龄小于37岁的男性,A2滑车部分打开的术后功能恢复相对较好(P<0.05)。结论M-Tang法肌腱缝合技术是修复Ⅱ区指深屈肌腱的简单、安全、有效方案,能满足肌腱早期保护性主动活动的需要。患者的年龄、性别、A2滑车切开与否对功能有明显影响。
Objective To analyze the clinical efficacy of M-Tang tendon suture technique for repair of flexor digitorum profundus tendon in zone Ⅱ. Methods From November 2013 to December 2016, 77 patients (96 fingers) with rupture of flexor digitorum profundus tendon in zone Ⅱ were treated with M-Tang tendon suture technique. The protective active and passive functional exercises were performed early after the operation. The range of active motion of the afiected finger was followed up and measured. The function of the affected finger was evaluated by Strickland and Glogovac criteria. The influencing Actors such as age, gender, A2 pulley treatment, concomitant injury, follow-up time and time from injury to operation were analyzed statistically. Results Nineteen cases were lost, while 58 cases (70 fingers) were follow-up for 4 to 27 months, with an average of 16 months. Among the 70 fingers, the results were rated as excellent in 43 fingers, good in 13 fingers, fair in 10 fingers and poor in 4 fingers. The excellent and good rate was 80%. No finger tendon rupture occurred. Among the patients, male patients younger than 37 years old had better functional recovery after partial opening of A2 pulley (P<0.05). Conclusion M-Tang tendon suture technique is a simple, safe and effective method for repairing flexor digitorum profundus tendon in zoneⅡ. It can meet the needs of early protective and active tendon movement. The age, gender and A2 pulley incision have significant influences on the treatment outcome.
