糜蛋白酶防止肌腱粘连的实验研究 被引量:4

摘要 目的探讨糜蛋白酶对肌腱粘连的影响。方法将36只健康白色纯种来享鸡,随机平均分成实验组和对照组。均以右侧第3趾为实验对象,切断趾深屈肌腱后采用改良Kessler法缝合。实验组向鞘管内滴注4 000 U/mL的糜蛋白酶1 mL,对照组采用生理盐水滴注。术后第2周、4周、6周处死动物,收集两组右侧缝合的趾深屈肌腱,分别进行大体观察,生物力学检测,组织学观察。结果实验组肌腱粘连程度较对照组明显减轻,在肌腱修复术后第4周和第6周肌腱的最大载荷也明显增强,差异有统计学意义(P<0.05)。结论糜蛋白酶有防止肌腱粘连的作用。 Objective To investigate the effect of chymotrypsin in decreasing the rate of post-operative adhesion. Methods 36 female healthy white leghorn chickens were randomly divided into experimental group and control group. The tendons of flexor profundus of 3rd claws were cut and repaired using modified Kessler method. Experi- mental group received exposure of chymotrypsin at a concentration of 4000 U/mL at tendon suture site, while the control group received a concentration of 0.9% saline solution. After 2.4.6 weeks following surgery, tendons from the 3rd toes were taken for gross, histological, and biomechanical examination. Results Compared with control group, the degree of adhesions of experimental group was significantly less. For biomechanical testing, the two groups have marked difference in maximum load 4.6 weeks after operation(P〈0.05). Conclusion A single intraoperative application of chymotrypsin with a concentration of 4000 U/mL appears to be an effective method for prevention of postoperative flexor tendon adhesion.
