几丁糖结合早期控制被动活动对屈指肌腱粘连的预防作用 被引量:11

摘要 目的:屈指肌腱损伤修复术后不可避免地形成粘连,为此探讨临床应用几丁糖结合早期控制被动活动预防屈指肌腱修复术后粘连的疗效。方法:对 60例 126指屈指肌腱损伤行改良 Kessler修复,术中于修复肌腱周围应用几丁糖,术后行早期控制被动活动,经 6~24个月随访,采用国际手外科联合会肌腱损伤委员会的评价方法进行疗效的评定。结果:手指总主动活动度优 90指 71.43%),良 24指 19.05%),可 6 ( (指(4.76%),差(4.76%),屈指肌腱功能恢复的优良率达 90.48%,没有发现肌腱断裂现象。结论:术中应用几丁糖结合早期控制被动活动可以预防屈指肌腱粘连。 AIM:All the restoration of injured flexor tendons will cause adhesion. The aim of this paper is to probe into the effect of chitosan and early controlled passive motion in preventing the adhesion of flexor tendons. METHODS: Totally 126 injured flexor tendons were repaired by the improved Kessler in 60 cases, chitosan was applied around the repaired tendons, and early controlled passive motion was performed after operation. The therapeutic effect was evaluated by the method determined by the tendon injury committee of the League of the International Hand Surgery after followed-up of 6 months to 24 months. RESULTS: The total active activity of fingers: 90 fingers were excellent(71.43%), 24 were better(19.05%), 6 were good(4.76%), and 6 were bad(4.76%).The excellentl recovery rate of the flexor tendons was 90.48%, no symptom of tendon rupture was found. CONCLUSION: The applying of chitosan and early controlled passive motion can prevent the adhesion of flexor tendons.
