摘要
目的采用美国心脏协会(american heart association, AHA)心血管急救技能培训中心的培训课程内容与方法,通过高质量的心肺复苏(cardiopulmonary resuscitation,CPR)技能培训,来提高医务人员基础生命支持(basiclife support,BLS)精准性。方法随机选取南京大学医学院附属鼓楼医院205名医务人员作为研究对象,采用AHA心血管急救技能培训中心的培训课程与方法进行培训。培训内容包括成人单人BLS、自动体外除颤器(automated external defibrillator, AED)和球囊面罩装置的使用、团队调动、儿童和婴儿BLS、窒息抢救。评价方式为培训前后进行理论和操作考核,培训后填写半结构式调查问卷,结果培训前学员理论考核平均得分为(78±15.7)分,培训后理论考核平均得分为(94.4±7.2)分,差异有统计学意义(P<0.001);培训前学员心肺复苏术操作综合评分平均为(58.3±21.7)分,达标率仅为54.6%,达到精准要求比例仅占2.4%,培训后综合评分为(83.1 ±9.1)分.达标率97.1%,达到精准要求比例占30.2%,差异有统计学意义(P<0.0001);培训后调查问卷显示学员对本中心培训模式的评价很高.认为所学内容有临床实用意义:结论经培训,医务人员BLS精准性得到大幅度提升,并掌握了指南更新内容,但精准性还可进一步提高,且医务人员掌握高质量急救技能的意识需强化。
Objective To improve the accuracy of basic life support (BLS) of medical staff through high-quality cardiopulmonary resuscitation (CPR) skills training and the content and methods of training courses referred to American Heart Association (AHA) training center. Methods A total of 205 medical staff from our hospital were randomly selected. The training courses and methods of the AHA Emergency Cardiovascular Skills Training Center were used for training. The training included the use of single-life rescuer adult BLS, automated external defibrillator (AED)and balloon mask devices, team mobility, BLS for children and infants, and suffocation rescue. The evaluation method was to conduct theoretical and operational assessment before and after the training, and filled in the semi-structured questionnaire after the training. Results The average score of the theoretical assessment of the trainees before training was 78±15.7 points. The average score of the theoretical assessment after training was 94.4±7.2 points, which was statistically significant (P<0.001). The average score of CPR operation before training was 58.3±21.7. The compliance rate was only 54.6%, and the accuracy requirement was only 2.4%. The comprehensive score after training was 83.1 ±9.1 points, the compliance rate was 97.1%, and the accuracy requirement was 30.2%, which was statistically significant (PvO.0001). The post-training questionnaire showed that the trainees have a high evaluation of the training mode of the center, and believed that the content of the study had clinical practical significance.Conclusion The BLS skills of medical staff was greatly improved, and the updated content of the guide was mastered after training. The awareness of medical personnel to master high-quality first-aid skills needs to be strengthened.
