摘要
目的探讨新生儿重症监护室(NICU)新生儿感染的特点和高危因素,就此提出相应目标性感染监测干预措施。方法选取我院2018年1—10月收治60例NICU新生儿为研究对象,将发生感染新生儿10例作为感染组,将未发生感染新生儿50例作为未感染组,分析NICU新生儿感染特点和高危因素。结果NICU新生儿以导管相关性感染最多,占比40.00%;共检出病原菌38株,排名前三位的分别是肺炎克雷伯菌(23.68%)、金黄色葡萄球菌(15.79%)、大肠埃希菌(13.15%);影响外周静脉置入中心静脉导管(PICC)感染的高危因素包括胎龄、出生体重、气管插管、PICC/脐静脉置管、NICU住院时间(P<0.05)。结论胎龄、出生体重、气管插管、PICC/脐静脉置管、NICU住院时间为影响其感染的高危因素,护理人员应结合高危因素的相关指标给予目标性监测干预措施,控制其医院感染发生率。
Objective To explore the neonatal intensive care unit(NICU) neonatal infection characteristics and high-risk factors and its corresponding target infection monitoring interventions. Methods To select 60 cases of NICU neonates who were treated in our hospital from January 2018 to October 2018 as the study subjects. We set the 10 cases of infected newborns as the infection group and 50 cases of non-infected newborns as the uninfected group.Then we analyzed the neonatal infection characteristics and high-risk factors. Results Among the NICU neonates,most of them had catheter-related infections,accounting for 40. 00%. We detected 38 strains of pathogens,with the top three of klebsiella pneumoniae(23. 68%),staphylococcus aureus(15.79%) and escherichia coli(13. 15%). The high-risk factors affecting peripherally inserted central venous catheters( PICC) infection include gestational age,birth weight,endotracheal intubation,PICC/umbilical venous catheterization,and NICU hospitalization( P < 0. 05). Conclusion The gestational age,birth weight,endotracheal intubation,PICC/umbilical venous catheterization,and NICU hospitalization are the high-risk factors affecting the infection. Therefore,nursing staff should give targeted monitoring interventions to control the incidence of nosocomial infections in accordance with the relevant indicators of high-risk factors.
