大同市急性呼吸道感染儿童肺炎链球菌的耐药性分析 被引量:1

摘要 目的探讨与分析大同市急性呼吸道感染儿童肺炎链球菌(SP)的耐药性。方法选择2017年3月至2018年12月大同市急性呼吸道感染患儿1 916例作为研究对象,调查患儿的临床资料,分离鉴定肺炎链球菌阳性情况并进行14种抗菌药物的耐药性分析。结果在1 916例患儿中,检出肺炎链球菌感染352例(肺炎链球菌组),检出率为18.3%。肺炎链球菌组的性别、年龄、发病季节、疾病类型、地区分布、血红蛋白、血小板计数等与非肺炎链球菌组对比差异无统计学意义(P>0.05)。肺炎链球菌组的白细胞计数、C反应蛋白与非肺炎链球菌组对比差异有统计学意义(P<0.05)。352株肺炎链球菌对万古霉素、利奈唑胺、左旋氧氟沙星全部敏感,对红霉素、克林霉素全部耐药,对甲氧苄啶、四环素等耐药率较高,对青霉素、头孢噻肟等耐药率较低。结论大同市急性呼吸道感染儿童SP感染比较常见,伴随有白细胞计数降低与C反应蛋白升高,对红霉素、克林霉素、甲氧苄啶、四环素等抗菌药物具有很高的耐药性。 Objective To investigate and determine the drug resistance of Streptococcus pneumonia(SP)in children with acute respiratory infection in Datong area.Methods A total of 1916 children diagnosed with respiratory tract infection between March 2017 and December 2018 in Datong area were included as the subjects in the study.The clinical data of the children were reviewed.The positivity of Streptococcus pneumoniae was determined by isola-tion and identification,and the drug resistance of 14 antibacterial drugs was analyzed.Results Among the 1916 children,352 were determined to have Streptococcus pneumoniae infection(Streptococcus pneumoniae group),with a detection yield of 18.3%.There were no statistically significant differences in gender,age,season of onset,disease type,regional distribution,hemoglobin and platelet counts between the Streptococcus pneumoniae group and the non-Streptococcus pneumoniae group(all P>0.05).There were statistically significant differences in white blood cell count and C-reactive protein level between the Streptococcus pneumoniae group and the non-Streptococcus pneumoniae group(both P<0.05).Two hundred Streptococcus pneumoniae strains were all sensitive to Vancomycin,Linezolid and Levofloxacin,all resistant to Erythromycin and Clindamycin,more likey to be resistant to Penicillin and Tetracycline,and less likely to be resistant to Amoxicillin and Cefotaxime.Conclusion Streptococcus pneumoniae infection is common in children in Datong area,showing lowered white blood cell count,elevated C-reactive protein level,and high resistance to antibacterials such as Erythromycin,Clindamycin,Trimethoprim and Tetracycline.
