摘要
目的观察并探讨早孕先兆流产患者采取不同剂量黄体酮治疗的临床效果。方法选取2017年8月—2018年8月期间530例来我院就诊的早孕先兆流产患者,所有患者给予黄体酮治疗,按照患者入院顺序随机分为实验组与对照组,每组患者人数各265例,对照组与实验组患者分别给予高剂量治疗与低剂量治疗,对两组患者的不良反应发生情况、稽留流产率,保胎成功率以及β-HCG水平、Prog值保胎成功率无显著差异进行观察与对比。结果实验组与对照组患者的血清β-HCG水平、血清Prog及保胎有效率无显著差异,P>0.05。实验组患者不良反应发生率及稽留流产率均显著低于对照组,差异具有统计学意义,P<0.05。结论不同剂量黄体酮治疗早孕先兆流产患者保胎效果无明显差异,其均能改善患者的妊娠结局,但若大剂量使用黄体酮会增加患者的稽留流产率及不良反应发生率。临床需根据患者病情选择剂量进行针对性治疗,从而降低不良妊娠结局发生率。
Objective To observe and investigate the clinical effect of different doses of progesterone in early pregnancy.Methods 530 cases of early pregnancy and threatened abortion patients from August 2017 to August 2018 were selected.All patients were given progesterone treatment.They were randomly divided into experimental groups and control groups according to the order of admission.265 cases,The patients in the control group and the experimental group were given high dose treatment and low dose treatment respectively.The occurrence of adverse reactions,retained abortion rate,β-HCG level,and Prog.values were observed and compared between the two groups.Results There was no significant difference in serumβ-HCG level,serum Prog and miscarriage between the experimental group and the control group,P>0.05.The incidence of adverse reactions and missed abortion rate in the experimental group were significantly lower than those in the control group,and the difference was statistically significant(P<0.05).Conclusion There is no significant difference in the effect of different doses of progesterone on patients with early pregnancy threatened abortion,which can improve the pregnancy outcome of patients,but the use of progesterone in large doses will increase the rate of missed abortion and adverse events.Therefore,the clinical need to select the dose according to the patient's condition for targeted treatment,thereby reducing the incidence of adverse pregnancy outcomes.
