摘要
目的观察甘精胰岛素联合口服降糖药对社区2型糖尿病患者的临床效果。方法选择卢沟桥社区卫生服务中心收治的二甲双胍和阿卡波糖治疗血糖控制不佳的2型糖尿病患者80例,随机分为观察组和对照组,每组40例。观察组采用甘精胰岛素联合二甲双胍和阿卡波糖治疗,对照组采用格列美脲联合二甲双胍和阿卡波糖治疗,患者均每周进行随访,持续观察3个月。监测患者治疗前后空腹血糖、餐后2 h血糖和糖化血红蛋白、体质量等指标。观察并记录患者的不良反应情况。结果治疗3个月后,2组空腹血糖、餐后2 h血糖、糖化血红蛋白均较治疗前降低,且观察组较对照组降低更明显(P <0. 05); 2组治疗后BMI、低血糖发生率比较,差异无统计学意义(P> 0. 05)。结论对于单纯口服药物治疗血糖控制不佳的社区2型糖尿病患者,正确使用相关甘精胰岛素药物,有助于针对血糖数值进行合理控制,不增加低血糖发生风险。
Objective To observe the clinical effect of insulin glargine combined with oral hypoglycemic drugs on type 2 diabetes mellitus in community. Methods Eighty patients with type 2 diabetes admitted to Lugouqiao Community Health Service Center were treated with metformin and acarbose, but the effect of blood sugar control was not good. They were randomly divided into observation group and control group with 40 cases in each group. The observation group was treated with insulin glargine combined with metformin and acarbose, while the control group was treated with glimepiride combined with metformin and acarbose. Weekly follow-up lasted for 3 months. The fasting blood glucose, 2 h postprandial blood glucose, glycosylated hemoglobin and body mass were observed before and after treatment. Observe and record the occurrence of adverse reactions. Results After 3 months of treatment, fasting blood glucose, 2 h postprandial blood glucose and glycosylated hemoglobin in the two groups were all lower than those before treatment, but the decrease in the observation group was more obvious than that in the control group ( P < 0. 05 ). There was no significant difference in the incidence of BMI and hypoglycemia between the two groups after treatment ( P > 0. 05). Conclusion For patients with type 2 diabetes mellitus in community who only receive medication but have poor blood sugar control, the correct use of insulin glargine is conducive to the rational control of blood sugar, and does not increase the risk of hypoglycemia, which is worthy of clinical application.
