摘要
目的探讨高同型半胱氨酸血症及微炎症与慢性肾脏病(CKD)患者血管内皮损伤的关系及其临床意义。方法选取2018年1—6月就诊于本院慢性肾脏病患者共90例,依据GFR分为3组,分别为GFR轻度下降组(GFR>60),GFR中度下降组(GFR 59.9~15),GFR重度下降组(<15)。另取同期来自本科室及门诊健康体检人员30名作为对照组(CN组)分别测定各组患者的s Cr、Hcy、CPR、v WF水平变化情况,采用Spearman等级相关分析用于两变量之间的关系。结果慢性肾脏病患者普遍存在高同型半胱氨酸血症、微炎症以及血管内皮损伤,其Hcy、CRP浓度显著高于对照组,且相关性分析显示各组Hcy、CRP与v WF呈正相关。结论 CKD患者普遍存在高同型半胱氨酸血症及微炎症,且Hcy与CRP是CKD患者血管内皮损伤的独立危险因素。
Objective To explore the relationship between hyperhomocysteinemia and micro-inflammation and vascular endothelial injury in CKD patients and its clinical significance. Methods A total of 90 patients with chronic kidney disease were selected from the first hospital affiliated to Jiamusi University. They were divided into 3 groups according to GFR,namely the mild decline group( GFR > 60),the moderate decline group( GFR 59.9-15) and the severe decline group( GFR < 15). SCr,Hcy,CPR and v WF were measured in each group.Spearman rank correlation analysis was used for the relationship between two variables. Results High hyperhomocysteinemia,micro-inflammation and vascular endothelial injury were common in patients with chronic kidney disease.The concentration of Hcy and CRP in patients with CKD was significantly higher than that of the control group.Correlation analysis showed that Hcy and CRP were positively correlated with v WF in each group.Conclusions Hyperhomocysteinemia and micro-inflammation are common in patients with CKD. Hcy and CRP are independent risk factors for vascular endothelial injury in patients with CKD.
