摘要
目的:探讨血液透析滤过治疗和血液灌流联合血液透析对维持性血液透析患者矿物质和骨代谢的影响。方法:选取2020年5月-2021年10月抚州市第一人民医院收治的90例行维持性血液透析治疗的患者作为研究对象。采用数字双盲分组法将患者分为试验组(n=48)和对照组(n=42)。试验组行血液灌流联合血液透析治疗,对照组行血液透析滤过治疗,两组均持续治疗6个月,比较两组预后。结果:试验组透析6个月后血肌酐(Scr)、尿素氮(BUN)及β2-微球蛋白(β2-MG)水平均低于对照组,差异均有统计学意义(P<0.05)。透析6个月后,试验组血钙和血磷水平均低于对照组,仅血磷水平差异有统计学意义(P<0.05)。试验组透析6个月后的甲状旁腺激素水平低于对照组,差异有统计学意义(P<0.05)。试验组透析6个月后的血清骨保护素(OPG)、骨形态发生蛋白-2(BMP-2)、骨碱性磷酸酶(BALP)水平均低于对照组,差异均有统计学意义(P<0.05)。结论:血液灌流联合血液透析模式能够更好地改善维持性血液透析患者的矿物质和骨代谢异常情况。
Objective:To investigate the effects of hemodiafiltration and hemoperfusion combined with hemodialysis on mineral and bone metabolism in maintenance hemodialysis patients.Method:A total of 90 patients receiving maintenance hemodialysis treatment in the First People's Hospital of Fuzhou from May 2020 to October 2021 were selected as the research objects.The patients were divided into the experimental group(n=48)and the control group(n=42)by digital double-blind grouping.The experimental group received hemoperfusion combined with hemodialysis,and the control group received hemodiafiltration.Both groups were treated for 6 months,and the prognosis of the two groups was compared.Result:The levels of serum creatinine(Scr),blood urea nitrogen(BUN)andβ2-microglobulin(β2-MG)in the experimental group were lower than those in the control group after 6 months of dialysis,the differences were statistically significant(P<0.05).After 6 months of dialysis,the blood calcium and blood phosphorus levels in the experimental group were lower than those in the control group,only the difference in blood phosphorus levels between the two groups was statistically significant(P<0.05).The level of parathyroid hormone after 6 months of dialysis in the experimental group was lower than that in the control group,the difference was statistically significant(P<0.05).The osteoprotegerin(OPG),bone morphogenetic protein-2(BMP-2)and bone alkaline phosphatase(BALP)levels in the experimental group after 6 months of dialysis were lower than those in the control group,the differences were statistically significant(P<0.05).Conclusion:The combination of hemoperfusion and hemodialysis mode can better improve the mineral and bone metabolism abnormalities in maintenance hemodialysis patients.
