摘要
目的分析硒酵母片联合甲巯咪唑片治疗对自身免疫性甲状腺病(AITD)伴甲状腺功能亢进患者甲状腺抗体及激素水平的影响。方法选取102例AITD伴甲状腺功能亢进患者为研究对象,按随机数字表法分为对照组与观察组,每组各51例。对照组口服甲巯咪唑片,观察组口服硒酵母片联合甲巯咪唑片,疗程均为12个月。比较两组疗效、治疗前后甲状腺抗体及激素水平。结果观察组治疗总有效率显著高于对照组,差异有统计学意义(90.20%vs 70.59%,P<0.05)。治疗后,两组血清游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)水平均较治疗前明显降低,甲状腺激素(TSH)水平均较治疗前明显升高,且观察组血清FT3、FT4水平均显著低于对照组,TSH水平显著高于对照组,差异均有统计学意义(P<0.05)。治疗后,对照组TPOAb、TGAb、TRAb等甲状腺抗体水平与治疗前相比均未见明显差异(P>0.05),而观察组甲状腺抗体水平较治疗前明显降低,且显著低于对照组(P<0.05)。结论硒酵母片联合甲巯咪唑片治疗AITD伴甲状腺功能亢进疗效确切,可有效减轻临床症状,缩小甲状腺体积,改善甲状腺功能,且可显著下调甲状腺抗体水平,值得推广。
Objective To analyze the effect of selenium yeast tablets combined with methimazole tablets on thyroid antibody and hormone levels in patients with autoimmune thyroid disease(AITD)and hyperthyroidism.Methods 102 cases of AITD with hyperthyroidism were collected and divided into control group and observation group according to random number table method,with 51 cases in each group.The control group were treated with methimazole tablets orally,and the observation group were treated with selenium yeast tablets combined with methimazole tablets orally for 12 months.The curative effects of two groups before and after treatment.were compared.Results The total effective rate of the observation group was significantly higher than that of the control group(90.20%vs 70.59%,P<0.05).After treatment,the levels of serum FT3 and FT4 in both groups were significantly lower than those before treatment,and the levels of TSH in observation group were significantly lower than those in control group(P<0.05).After treatment,the levels of TPOAb,TGAb,TRAb and other thyroid antibodies in the control group were not significantly different from those before treatment(P>0.05),while the levels of thyroid antibodies in the observation group were significantly lower than those before treatment,and were significantly lower than those in the control group(P<0.05).Conclusion Selenium yeast tablets combined with methimazole tablets is effective in the treatment of AITD with hyperthyroidism.It can effectively alleviate clinical symptoms,reduce thyroid volume,improve thyroid function,and significantly reduce the level of thyroid antibody.
