摘要
目的探讨丁苯酞联合阿替普酶对急性脑梗死患者血清炎性因子及神经功能的影响。方法选择我院收治的急性脑梗死患者118例,随机分为两组,每组59例。对照组给予阿替普酶静脉溶栓治疗,观察组在此基础上口服丁苯酞软胶囊治疗。比较两组患者临床疗效、炎性因子、基质金属蛋白酶(MMP-9)、血管内皮生长因子(VEGF)水平及神经功能情况。结果观察组总有效率为93.22%高于对照组的79.66%,差异有统计学意义(P<0.05);治疗14 d,两组白介素-6(IL-6)、白介素-8(IL-8)、C反应蛋白(CRP)及MMP-9下降,VEGF升高;治疗90 d后,美国国立卫生院卒中量表(NIHSS)评分降低,Barthel指数(BI)升高,观察组上述各指标均优于对照组,差异有统计学意义(P<0.05)。结论丁苯酞联合阿替普酶治疗急性脑梗死具有较好的临床疗效,可减少患者神经功能缺损,降低炎性反应,值得临床推广。
Objective To explore the effects of butylphthalide combined with alteplase on serum inflammatory factors and neurological function of patients with acute cerebral infarction.Methods A total of 118 patients with acute cerebral infarction were enrolled and randomly divided into two groups,59 cases in each group.The control group was treated with alteplase and the observation group was treated with butylphthalide on the basis of the control group.The clinical efficacy,inflammatory factors,matrix metalloproteinase(MMP-9),vascular endothelial growth factor(VEGF)levels and neurological function were compared between the two groups.Results The total effective rate of the observation group(93.22%)was higher than that of the control group(79.66%)and the difference was statistically significant(P<0.05).After 2 weeks of treatment,the interleukin-6(IL-6),interleukin-8(IL-8),C-reactive protein(CRP)and MMP-9 of the two groups decreased and the VEGF increased.After 90 days of treatment,the National Institutes of Health Stroke Scale(NIHSS)score decreased and Barthel index(BI)score increased.The above indicators of the observation group were all beteer than those of the control group and the difference was statistically significant(P<0.05).Conclusion Butylphthalide combined with alteplase has significant clinical efficacy in the treatment of acute cerebral infarction,which can reduce neurological deficits and reduce inflammatory response.
