摘要
目的:调查北京市某三级老年医院护士道德困境现状,分析其影响因素,旨在为护理管理者采取针对性的道德困境干预措施提供参考依据。方法:采用方便抽样方法,选取北京市某三级老年医院临床科室的291名护士,采用中文版护士道德困境量表进行调查。结果:护士道德困境总分为30.00(13.00,56.00)分,道德困境频率条目均分(0.87±0.63)分,强度条目均分(0.82±0.49)分,显示护士道德困境处于较低水平。职称、对护理工作是否满意、护理工作是否充满意义和价值,以及在工作中是否感到快乐影响护士道德困境水平(P<0.01)。结论:老年医院护士的道德困境处于相对较低水平。护理管理者应及时与护士进行有效沟通,了解护士在工作中遭遇的道德困境情况,优化护理人力资源配置,改变教育理念,培养具有法律知识和伦理意识的护理人才,从而缓解护士道德困境程度。
Objective: To investigate moral distress among nurses in a tertiary geriatric hospital in Beijing and explore the influencing factors, and to provide reference for nursing managers to adopt targeted interventions for moral dilemma.Methods: A convenience sample of 291 nurses from clinical departments in a tertiary geriatric hospital in Beijing were investigated with the Chinese version Moral Distress Scale-revised(MDS). Results: The total score of MDS was 30.00(13.00,56.00), and the scores of moral distress frequency and moral distress intensity were 0.87±0.63 and 0.82±0.49, respectively,which meant a lower level of nurses’ moral distress. Professional title, nurses’ job satisfaction, the meaning and value of job,and job-related well-being were significantly correlated with nurses’ moral distress(P<0.05). Conclusion: Nurses in geriatric hospital perceived a lower level of moral distress. Communicating with nurses timely, knowing nurses’ moral dilemmas in their daily work, better allocation of nursing human resource and changing education concept to cultivate practical nurse with legal knowledge and ethical consciousness are benefit to reduce nurses’ moral dilemmas.
