基于多频生物电阻抗法的分层级护理干预对腹膜透析患者容量管理的影响 被引量:8

Effects of hierarchical nursing intervention based on multi-frequency bioelectrical impedance on volume management in patients receiving peritoneal dialysis

摘要 目的观察基于多频生物电阻抗法的分层级护理干预对腹膜透析患者容量管理的影响。方法采用便利抽样法,选取2016 年2 月—2018 年2 月于郑州大学第一附属医院进行腹膜透析的患者 64 例为研究对象,在常规护理基础上每周进行多频生物电阻抗测定,根据测定结果采取分层级护理干预12 周,比较干预前后体重、BMI、干体重、干体重达标率、身体总水量、细胞外液量、容量平衡率、血压、血压达标率、浮肿指数、浮肿指数达标率、OH值等。结果 64 例患者干预12 周后血压达标率、容量平衡率、干体重达标率、浮肿指数达标率均较干预前升高,OH值、收缩压、舒张压、体重、BMI、细胞外水分、身体总水分、浮肿指数、干体重均较干预前降低,差异均有统计学意义(P<0.05)。结论基于多频生物电阻抗法的分层级护理干预可以有效管理腹膜透析患者的容量问题,提高干体重达标率及改善患者血压。 Objective To observe the effects of hierarchical nursing intervention based on multi-frequency bioelectrical impedance on volume management in patients receiving peritoneal dialysis. Methods Totally 64 patients who received peritoneal dialysis in the First Affiliated Hospital of Zhengzhou University from February 2016 to February 2018 were selected by convenient sampling. Multi-frequency bioelectrical impedance was measured on the basis of conventional nursing care. Hierarchical nursing interventions were provided for 12 weeks according to the measurement results. The body weight, body mass index( BMI), dry weight, dry weight compliance rate, total body water( TBW), extracellular water( ECW), volume balance ratio, blood pressure, blood pressure compliance rate, dropsy index, dropsy index compliance rate and OH value before and after the intervention were compared between the two groups. Results After 12 weeks, the 64 patients' blood pressure compliance rate, volume balance ratio, dry weight compliance rate and dropsy index compliance rate rose compared with the levels before the intervention, while OH value, systolic pressure, diastolic pressure, body weight, BMI, ECW, TBW, dropsy index and dry weight decreased compared with the levels before the intervention(P<0.05). Conclusions Hierarchical nursing intervention based on multi-frequency bioelectrical impedance can effectively manage the volume in patients receiving peritoneal dialysis, increase their dry weight and improve their blood pressure.
