高血压患者血浆致动脉粥样硬化指数水平的变化及其相关因素分析 被引量:10

摘要 目的探讨高血压患者血浆致动脉粥样硬化指数(atherogenic index of plasma,AIP)水平的变化及其与腹部内脏脂肪指标的相关性。方法选取2013年3月~2014年12月广西柳江县高血压患病率调查人群中原发性高血压患者207例,按照是否伴AIP增高分为两个亚组,即AIP增高组(AIP≥0.06,131例)、AIP正常组(AIP <0.06,76例)。另选取同期调查人群非高血压者217例作为正常对照组。测量受试者身高、体重、腰围(waist circumference,WC)、血压,检测空腹血脂、血糖,采用生物电阻抗分析方法测量其内脏脂肪指数(visceral fat index,VAI)、身体脂肪率(percentage body fat,PBF),并计算体质量指数(body mass index,BMI)、腰围身高比(waist height ratio,WHt R)及AIP。分析各生化、内脏脂肪测量指标与AIP的相关关系。结果高血压组AIP水平显著高于正常对照组,差异有统计学意义(P <0.05)。高血压组AIP值水平与腰围、WHtR、PBF、VAI呈正相关(r=0.426、0.371、0.155、0.452,P <0.05),与性别、年龄、BMI、收缩压、舒张压、空腹血糖等均无明显相关性(P> 0.05)。logistic回归分析结果显示,VAI、WC是AIP值的影响因素。结论广西柳江县地区高血压患者AIP水平显著高于非高血压人群,AIP值水平与WC、WHr T、PBF、VAI有显著相关性,VAI、WC是AIP值增高的影响因素。 Objective To investigate the varying levels of atherogenic index of plasma(AIP) in hypertensive patients and its factors associated. Methods 207 patients with primery hypertensive were selected from theepidemiological survey database from Guangxi liujiang from March 2013 to December 2014 as the patient group. In the same database,217 cases with non- primery hypertensive is the control group. According to the levels of AIP(≥ 0.06),207 patients with primery hypertensive were divided into two groups. There were 131 samples in hypertensive-high AIP group and 76 cases in hypertensive-nomal AIP group. Visceral adiposity Index(VAI) was detected by bioelectrical impedance analysis. In addition,we record the following indicators in each group:sex,age,height,weight,waist circumference(WC),systolic blood pressure,diastolic blood pressure,fasting blood glucose,blood fat and calculate Body mass index(BMI),Waist height ratio(WHtR),Body fat rate(BFP),Atherogenic Index of Plasma(AIP). Analysis of two groups of various clinical and biochemical index differences. Results The levels of AIPof hypertension group were significantly higher than that of control group(P < 0.05). AIP level and WC,WHtR,BFP,VAI were positively correlated(r value are:0.426,0.371,0.155,0.452,P < 0.05). With gender,age,BMI,systolic pressure,diastolic blood pressure,fasting glucose have no significant correlation(P > 0.05). Multi-factor stepwise regression analysis results showed that VAI,WC is a risk factor for AIP.Conclusions (1) The levels of AIP of hypertension group is higher significantly than the control group.(2) The levels of atherogenic index of plasma has a positive relation with VAI and WC.
