摘要
目的观察过敏性紫癜(HSP)患儿血清细胞因子白细胞介素-21(IL-21)、肿瘤坏死因子-α(TNF-α)、转化生长因子(TGF-β1)和免疫球蛋白(Ig A1、IgA)的水平,并探讨其临床意义。方法将我院收治的56例急性期HSP患儿根据尿常规结果分为HSP组(n=32)和紫癜性肾炎(HSPN)组(n=24);另选取同时间段在我院体检的30名健康儿童作为对照组。检测并比较三组研究对象的血清IL-21、TNF-α、TGF-β1、Ig A1及IgA水平,并对HSP组患儿血清细胞因子及免疫球蛋白水平进行Pearson相关性分析。结果 HSP组和HSPN组的血清IL-21水平显著低于对照组,TNF-α、TGF-β1水平显著高于对照组(P<0.05);HSPN组的血清TNF-α、TGF-β1水平显著高于HSP组(P<0.05)。HSP组和HSPN组的IgA1、IgA水平显著高于对照组(P<0.05);HSPN组的IgA水平显著高于HSP组(P<0.05)。Pearson相关性分析结果显示,TNF-α与TGF-β1、IgA1均呈正相关,IgA1与TGF-β1、IgA均呈正相关(P<0.05)。结论血清IL-21、TNF-α、TGF-β1、IgA1及IgA水平均与HSP的发生、发展密切相关,检测相关指标水平并及早采取措施,可有效预防疾病的发生及恶化。
Objective To observe the levels of serum cytokines interleukin-21(IL-21), tumor necrosis factor-α(TNF-α), transforming growth factor-β1(TGF-β1) and immunoglobulin(IgA1, IgA) in children with Henoch-Schonlein purpura(HSP) and explore the clinical significance. Methods Fifty-six children with acute HSP treated in our hospital were divided into HSP group(n=32) and Henoch-Schonlein purpura nephritis(HSPN) group(n=24) according to the results of urinary routine. Thirty healthy children who had physical examination in our hospital at the same time were selected as control group. The levels of serum IL-21, TNF-α, TGF-β1, IgA1 and IgA were detected and compared among the three groups. Pearson correlation analysis of serum levels of cytokines and immunoglobulins was performed in the HSP group. Results The levels of serum IL-21 in the HSP group and HSPN group were significantly lower than those in the control group, the levels of TNF-α and TGF-β1 were significantly higher than those in the control group(P <0.05). The levels of TNF-α and TGF-β1 in the HSPN group were significantly higher than those in the HSP group(P<0.05). The levels of IgA1 and IgA in the HSP group and HSPN group were significantly higher than those in the control group(P<0.05), and IgA level in the HSPN group was significantly higher than that in the HSP group(P<0.05). The results of Pearson correlation analysis showed that the level of TNF-α was positively correlated with the level of TGF-β1 and IgA1, and the level of IgA1 was positively correlated with the level of TGF-β1 and IgA(P<0.05). Conclusion The levels of serum IL-21,TNF-α, TGF-β1, IgA1 and IgA are closely related to the occurrence and development of HSP.Detecting the levels of relevant indicators and taking early measures can effectively prevent the occurrence and deterioration of the diseases.
