摘要
目的探讨心电R波脉搏波传导速度(PWV)在高血压动脉硬化中的临床应用价值。方法选择2015年1月～2017年1月在赣州市立医院心内科门诊或住院部就诊的210例原发性高血压患者作为研究对象,所有患者均行颈动脉彩色多普勒超声检测以及计算心电R波PWV,根据颈动脉是否形成粥样硬化斑块,将其分为硬化组(56例)与正常组(154例),比较两组患者PWV的差异性,推算其诊断动脉硬化的阈值、灵敏度、特异度、阳性似然比、阴性似然比。结果硬化组的心电R波PWV明显快于正常组,差异有统计学意义(P<0.05)。心电R波PWV诊断颈动脉粥样硬化的最佳阈值为4.500 m/s,其灵敏度为92.86%,特异度为90.91%,阳性似然比为10.22,阴性似然比为0.08。结论心电R波脉搏传导速度对于高血压颈动脉粥样硬化的诊断具有重要应用价值,建议将4.500 m/s作为心电R波PWV的诊断阈值。
Objective To investigate the clinical application value of ECG R wave pulse wave velocity(PWV)in hypertensive arteriosclerosis.Methods A total of 210 patients with essential hypertension who were admitted to the Department of Cardiology or Inpatient Department of municipal hospital from January 2015 to January 2017 were selected as the research objects.All patients underwent carotid color Doppler ultrasonography and calculation ECG R wave PWV,according to whether the carotid artery formed atherosclerotic plaque,they were divided into the sclerosing group(56 cases)and the normal group(154 cases).The difference of PWV between the two groups was compared,and the threshold of diagnosis of arteriosclerosis,sensitivity,specificity,positive likelihood ratio,and negative likelihood ratio were calculated.Results The conduction velocity of ECG R wave PWV in the sclerosis group was significantly faster than that in the normal group,and the difference was statistically significant(P<0.05).The optimal threshold for diagnosis of carotid atherosclerosis by ECG R wave PWV was 4.500 m/s,the sensitivity was 92.86%,the specificity was 90.91%,the positive likelihood ratio was 10.22,and the negative likelihood ratio was 0.08.Conclusion ECG R wave PWV has important application value in the diagnosis of hypertensive carotid atherosclerosis.It is recommended to use 4.500 m/s as the diagnostic threshold for ECG R wave PWV.
