摘要
目的观察H型高血压患者血管内皮功能及脉搏波传导速度的变化。方法选择2015年9月~2015年12月于阳江市人民医院收治的入院诊断为原发性高血压患者250例,其中男性124例,女性126例。根据入院同型半胱氨酸(Hcy)水平将患者分为H型高血压组200例(Hcy≥10μmol/L)与对照组50例(Hcy<10μmol/L)。入组所有患者在入院第2 d完善以下检验指标,脉搏波传导速度(PWV);血流介导的血管舒张反应(FMD)、硝酸甘油介导的血管舒张反应(NMD);高敏C反应蛋白(hs-CRP)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、血糖(GLU)、丙氨酸转氨酶(ALT)、肌酐(Cr)。比较两组患者各检验指标有无差异,并将H型高血压组患者Hcy水平与hs-CRP、FMD、NMD、PWV水平进行相关性分析。结果与对照组相比,H型高血压组患者ALT、CR、GLU、TC、LDL无明显差异(P>0.05),hs-CRP、PWV水平明显升高(P<0.05),FMD、NMD降低(P<0.05)。相关性分析结果表明Hcy与ba-PWV呈正相关(P<0.05),与FMD、NMD呈负相关(P<0.05),Hcy与hs-CRP无明显相关性(P>0.05)。结论与普通原发性高血压相比,H型高血压患者血管内皮功能损害更严重,PWV水平更高,且这种变化随着Hcy的升高更加明显。
Objective To investigate vascular endothelial function and pulse wave velocity(PWV)in patients with H-type hypertension.Methods From2015.9to2015.12,250primary hypertension patients were included in this study.There were124males and126females.Based on level of homocysteine(Hcy),the patients were divided into control group(Hcy<10μmol/l)(50patients)and H-type hypertension group(Hcy≥10μmol/l)(200patients).PWV,flow-mediated dilatation(FMD),Nitroglycerin-mediated vasodilatation(NMD).high-sensitivity C-reactive protein(hs-CRP),total cholesterol(TC).low density lipoprotein cholesterol(LDL-C),blood glucose(GLU),alanine aminotransferase(ALT)and creatinine(Cr)were measured on the second day after admission.Results There was no difference of ALT,CR,GLU,TC,LDL between the two groups(P>0.05).The levels of hs-CRP and PWV in H-type hypertension group were significantly higher than those in control group(P<0.05).FMD and NMD were lower in H-type hypertension group than those in control group(P<0.05).The correlation analysis results showed that Hcy was positively correlated with ba-PWV(P<0.05),and negatively correlated with FMD and NMD(P<0.05).Hcy and hs-CRP had no significant correlation(P>0.05).Conclusion H-type hypertension patients have more serious vascular endothelial dysfunction and higher level of PWV,and this change is more obvious with the increase of Hcy.
