摘要
目的探讨分析丙戊酸钠在临床上引起的不良反应特点。方法选取某院神经内科(门诊+住院)2011年6月至2017年6月服用丙戊酸钠抗癫痫治疗出现不良反应的40例患者为研究对象,分别从患者年龄、性别、用药情况、服药后出现不良反应的时间及其临床表现等方面进行分析。结果丙戊酸钠引发的不良反应可发生于各个年龄段,10岁以下儿童相对较多,性别无明显差异,与药物剂量呈正相关性,于服药后出现不良反应时间为1 d～2年。临床表现为中枢神经系统不良反应16例(40.0%),消化系统不良反应9例(22.5%),皮肤及皮下组织不良反应8例(20.0%),血液系统不良反应5例(12.5%),其他不良反应2例(5.0%)。结论丙戊酸钠导致的不良反应临床表现多样,可同时累及多个系统。临床医务人员要重视丙戊酸钠药物引起的不良反应,选定丙戊酸钠治疗相关疾病时应定期检测患者肝功能、血常规、血药浓度,积极预防不良反应发生。
Objective To investigate and analyze the characteristics of clinical adverse effects of sodium valproate.Methods A total of 40 patients who had adverse reactions to the treatment of epilepsy with sodium valproate during the period from June 2011 to June 2017 in the Department of Neurology of a hospital(patient outpatient+hospitalization)were selected.The patients’age,sex,medication,the time and clinical manifestations of adverse reactions after taking the drug were analyzed.Results Adverse reactions caused by sodium valproate occurred in all age groups,with relatively more among children under 10.There was no significant difference between sexes and a positive correlation with drug dose.The occurrence of adverse reactions after taking the drug was 1 day to 2 years.The clinical manifestations of adverse reactions were in the central nervous system [16 cases(40.0%)],adverse reactions in the digestive system[9 cases(22.5%)],adverse reactions in the skin and subcutaneous tissues[8 cases(20.0%)],adverse reactions in blood system[5 cases(12.5%)] and other adverse reactions[2 cases(5.0%)].Conclusion The clinical manifestations of sodium valproate-induced adverse reactions were diverse and could affect multiple systems at the same time.Clinical medical personnel should pay attention to the adverse reactions caused by sodium valproate drugs.When sodium valproate is used for the treatment of related diseases,liver function,blood routine and plasma concentration of patients should be regularly tested to prevent the occurrence of adverse reactions.
