摘要
目的探讨甲基苯丙胺(冰毒)依赖者脑血流动力学的变化。方法纳入27名新入强制戒毒所甲基苯丙胺依赖者和23名健康对照者作为研究对象,使用经颅多普勒(TCD)仪检测各脑血管的相关血流参数,分析其结果。甲基苯丙胺依赖者需分别在入所3 d内及戒断至28~30 d进行检查。结果甲基苯丙胺依赖者戒断前大脑中动脉、大脑前动脉的Vp、Vm及PI高于对照组(P<0.05),大脑后动脉的Vp和PI高于对照组(P<0.05),椎动脉的PI高于对照组(P<0.05),基底动脉各项血流数值与对照组差异无统计学意义(P>0.05);甲基苯丙胺依赖者戒断后大脑中动脉的Vp和PI低于戒断前(P<0.05),戒断后大脑前动脉的Vp、Vm及PI低于戒断前(P<0.05),戒断后大脑后动脉的Vp低于戒断前(P<0.05),戒断后椎动脉及基底动脉各项血流数值与戒断前差异无统计学意义(P>0.05)。结论长期慢性使用甲基苯丙胺会导致脑血流动力学的异常,但经过一段时间的戒断后可得到改善。
Objective To investigate the changes of cerebral blood flow velocity in methamphetamine(MA) dependent subjects. Methods We recorded cerebral blood flow velocity by transcranial Doppler(TCD) in new entry into the drug detoxification station methamphetamine dependent subjects(n=27) and in control subjects(n=23) to analyze the results. Blood flow velocity was recorded within 3 days of admission and 28 to 30 days of monitored abstinence. Results Pulsatility index(PI), peak sys tolic velocity(Vp) and mean velocity(Vm) were significantly increased in the methamphetamine users vs control subjects in the anterior(ACA) and middle(MCA) cerebral arteries(P<0.05), PI and Vp were significantly increased in the posterior(PCA) cerebral artery(P<0.05),PI were significantly increased in the vertebral(VA) cerebral artery(P<0.05),the blood flow velocity was no significant difference in basilar(BA) artery(P>0.05).Methamphetamine users had decreased Vp and PI in the MCA, decreased Vp,Vm and PI in the ACA, decreased Vp in the PCA after a month of monitored abstinence(P<0.05), the blood flow velocity was no significant difference in VA and BA(P>0.05). Conclusion Long-term chronic use of methamphetamine can cause abnormalities in cerebral blood flow velocity, but can be improved after a period of withdrawal.
