摘要
目的:探讨三维斑点追踪成像(3D-STI)与实时三维超声心动图技术(RT-3DE)评价冠心病患者(CHD组)左心室收缩同步性应用价值及其与收缩功能的相关性。方法:32例(A组:单支或多支50%<狭窄≤70%)、28例(B组:单支或多支狭窄>70%)和45例健康体检者(对照组)行RT-3DE和3D-ST检查,获得左心室舒张末期容积(EDV)、收缩末期容积(ESV)和左室射血分数(LVEF),16、12节段的纵向、径向、环向应变收缩峰值时间的标准差(TLS、TRS、TCS)以及与最小收缩容积时间的时间之差(TLSV、TRSV、TCSV),左心室16、12、6心肌收缩期最小容积达峰时间标准差(TesvSD)和最大差值(Tesv-Dif)。结果:A组Tesv-Dif、Tesv-16、Tesv-12、Tesv-6、TLS-16、TCS、TLSV-16较对照组延长,差异有统计学意义(P<0.05);B组上述时间指标较对照组明显延长(P<0.01),且与A组比较差异均有统计学意义(P<0.05)。RT-3DE与3D-STI同步性参数相关性分析示:LVEF与Tesv-16、TLS-16呈负相关(r=-0.702、-0.638,P<0.01),而Tesv-16与TLS-16呈正相关(r=0.603,P<0.01)。结论:RT-3DE与3D-STI可客观评价冠心病患者左室收缩同步性,尤其是患者早期单支或多支50%<狭窄≤70%,且二者有一定的相关性。
Objective:To explore the value of 3D speckle Tracking Imaging(3D-STI)and real-time three-dimensional echocardiography(RT-3DE)in evaluating left ventricular systolic synchrony in patients with coronary heart disease(CHD Group)and its correlation with systolic function.Method:RT-3DE and 3D-ST were performed in 32 patients(group A:single or multiple vessel 50%<stenosis≤70%),28 patients(group B:single or multiple vessel stenosis>70%)and 45 healthy subjects(control group).The left ventricular end-diastolic volume(EDV),end-systolic volume(ESV)and left ventricular ejection fraction(LVEF),the standard deviation of the peak time of longitudinal,radial and circumferential strain(TLS,TRS)of 16,12 segments were obtained.TCS,TLSV,TRSV and TCSV,Tesv-SD and Tesv-Dif of the minimum systolic volume of left ventricle 16,12 and 6.Result:Tesv-Dif,Tesv-16,Tesv-12,Tesv-6,TLS-16,TCS and TLSV-16 in group A were prolonged compared with the control group(P<0.05).The timing indicators of group B were significantly prolonged than those of the control group(P<0.01),compared with group A,there were statistically significant differences(P<0.05).In the correlation analysis between RT-3DE and 3D-STI synchronicity parameters,Tesv-16 and TLS-16 had negative correlation with LVEF(r=0.702,-0.638;P<0.01).Tesv-16 was positively correlated with TLS-16(r=0.603,P<0.01).Conclusion:RT-3DE and 3D-STI can objectively evaluate left ventricular systolic synchrony in patients with coronary heart disease,especially in patients with single or multiple branches 50%<stenosis≤70%in the early stage,and there had a certain correlation between them.
