摘要
目的通过对某三甲医院内外科系统影响患者出院结账因素比较分析,找出其中的差异,探讨改善出院结账延迟的有效措施。方法对3 586名出院患者出院结账时进行调查统计,采用卡方检验分析内外科整体及不同人员间影响患者出院结账因素差异。结果①内外科患者出院结账受影响百分率存在差异(P<0.001),外科区患者出院结账受影响百分率大于内科区;②内外科均因医师方面导致患者出院结账延迟比例最高,且影响因素在不同人员分布间也存在明显差异(P<0.001),外科区因药房(P<0.001)和信息处(P<0.001)导致患者出院结账延迟比例较高,而内科区因检验功能科(P<0.001)和患者自身因素(P<0.001)导致患者出院结账延迟比例较高;③经"8020"原则进行分析,内外科系统影响患者出院结账因素真因存在差异。结论由于内外科疾病特点及诊疗过程的差异,内外科影响患者出院结账因素存在明显差异,医院应采取更有针对性的措施,改善内外科出院结账延迟。
Objective To compare and analyze the factors influencing the patients’discharge and settlement in the internal and surgery system of a third-grade class-A hospital,find out the differences,and explore the effective measures to improve the delay of discharge and settlement.Methods A total of 3,586 discharged patients were investigated when they were discharged from the hospital.Chi-square test was used to analyze the difference of factors affecting patients’ discharge and settlement between the whole internal surgery department and different personnel.Results ①There was a difference in the percentage of patients affected by discharge settlement of internal medicine and surgery department(P<0.001),and the number of surgery department was larger than that of internal medicine department.② Both departments had the highest percentage of patients discharged from hospital check-out delay caused by doctors,and the influencing factors among the different personnel distribution had significant difference (P<0.001);for pharmacy (P<0.001) and the infoshop (P<0.001) in patients discharged from hospital check-out delay ratios were higher in surgical area,while for inspection function (P<0.001) and the patient’s own factors (P<0.001) was higher in internal area.③As for the analysis of the 8020 principle,there were differences in the factors affecting patients’ discharge and settlement of hospital discharge caused by the internal surgical system.Conclusion Due to differences in local disease characteristics and the diagnosis and treatment process,there exists significant difference of factors affecting patients discharged from hospital,and more targeted measures should be taken to improve discharge check-out delay.
