摘要
目的建立儿童医院门诊签到制就诊系统,并评价其在门诊临床中的应用效果。方法选取2018年6月11日至2018年6月13日来我院"名医馆"诊区就诊的1917例患者为研究对象,比较患者挂号单或手机通知单上的建议就诊时间+3min(t1)和患者签到时间(t2)。t1<t2时,说明患者就诊迟到,查看迟到不同时间段的占比率。结果研究结果显示t1<t2共有449例,其中t2-t1在1～30 min有239例;t2-t1在30～60 min有101例;t2-t1在60～90 min有58例;t2-t1在90 min以上有51例。结论门诊签到制就诊系统实施后,未签到的患者,医生呼叫器端不予显示,医生呼叫直接跳到下一个已签到患者,门诊医生叫空率下降20%以上。诊区护士省去给迟到患者刷卡排入呼号队列的过程,工作量减少,医生、护士有更多的时间服务于问诊患者,提升了门诊患者的护理质量及护理满意度、医生满意度、患者就诊满意度。
Objective To establish the outpatient registration system in children’s hospital and evaluate its clinical application. Methods A total of 1917 patients who visited the “Famous Doctor” of our hospital on June 11, 2018 to June 13, 2018 were selected as research objects. The recommended visit time on the patient registration form or mobile phone notification form +3 min (t1) and patient check-in time (t2) were compared. When t1<t2, it indicated that the patient was late for treatment and the proportion of late in different time periods was checked. Results AThe results showed that there were 449 cases with t1<t2, including 239 cases with t2-t1 within 1-30 min, 101 cases with t2-t1 within 30-60 min, 58 cases with t2-t1 within 60-90 min, and 51 cases with t2-t1 over 90 min. Conclusion After the implementation of outpatient sign-in system, for patients who did not sign in, the pager end of the doctor did not be displayed, and the pager directly jumped to the next patient who had signed in, and the vacancy rate of the outpatient doctor dropped by more than 20%. The nurse in the diagnosis area saves the process of swiping the card and lining up the call number for the patients who are late, thus the workload is reduced. The doctors and nurses have more time to serve patients in consultation, which improves the quality of outpatient care and nursing satisfaction, doctor satisfaction and patient satisfaction.
