摘要
目的:通过实验分析糖尿病肾病(Ⅳ、Ⅴ期)中医证候,准确判断糖尿病肾病患者的证候并对症诊治。方法:选取2015年5月-2017年5月某院内分泌科、肾病及保健科住院患者共100例进行实验研究,探讨糖尿病肾病的中医证候特点,分析患者的病因病机,深化患者病症与证候之间的关系,有效提升糖尿病肾病(Ⅳ、Ⅴ期)患者中医辨证分型的标准化、客观化,同时为糖尿病肾病患者的防治提供积极借鉴意义。结果:所选100例糖尿病肾病患者中,IV期62例,占62%,V期38例,占38%;IV期患者常出现的症状包括全身浮肿、大便干、小便黄且量多、口干口黏、口渴多饮、肢体麻木、神疲乏力以及少气懒言等,中医证型包括气虚、血虚以及阴阳两虚等虚证,实证包括血瘀、痰浊及水湿证;V期患者主要症状包括肢体麻木、畏寒肢冷、口干、面足浮肿及神疲乏力,中医证候主要包括气虚、阴虚、血瘀等,且血瘀证明显增加。结论:中医证候能够作为糖尿病肾病患者的临床诊断之一,确保中医证候分型的标准化、客观化能够为糖尿病肾病患者临床治疗提供积极借鉴意义,值得推广应用。
Objective:To analyze the TCM syndromes of diabetic nephropathy(phase Ⅳ and Ⅴ),accurately judge the symptoms of diabetic nephropathy and diagnose.Methods:100 patients were selected.The characteristics of TCM syndromes of diabetic nephropathy was explored.The etiology and pathogenesis ware analyzed.The relationship between illness and syndromes was deeped.Standardization and objectification of TCM syndrome differentiation were improved,and positive reference for the prevention and treatment of diabetic nephropathy was provided.Results:In the 100 patients,62 cases were phase Ⅳ,accounting for 62%.38 cases were phase Ⅴ,for 38%.The symptom of phase Ⅳ includes anasarca,constipate,yellow and more of pee,dry mouth and sticks mouth,thirsty and much drinking,numbness of the limbs,lacking in strength and distinclination to talk.TCM syndromes include the Qixu(气虚),Tanzhuo(痰浊)and Shuishi(水湿)type.The symptom of phase Ⅴ includes numbness of the limbs,chills,dry mouth,edema and lacking in strength.TCM syndromes mainly include the Qixu,Yinxu(阴虚)and Xueyu(血瘀)type.Conclusion:TCM syndromes can be used as one of the clinical diagnosis of diabetic nephropathy.The standardization and objectivity of TCM syndromes can provide positive reference for the clinical treatment,and is worth popularizing.
