摘要
目的探讨水通道蛋白4(AQP4)抗体、髓鞘少突胶质细胞糖蛋白抗体(MOG)抗体阳性NMOSD患者的视网膜微观结构损伤的特点。方法收集新疆维吾尔自治区人民医院诊治的27例NMOSD患者,同时选择20例年龄、性别相匹配的健康志愿者作为对照组。采用光学相干断层成像(OCT)检测NMOSD组及健康对照组视网膜神经纤维层(RNFL)12个钟点位的厚度,对AQP-4抗体阳性NMOSD、MOG抗体阳性NMOSD各钟点位RNFL进行比较分析。结果与健康对照组比较,NMOSD病例组RNFL层厚度在1点钟位、6点钟位、7点钟位、8点钟点位、9点钟位、10点钟位、11点钟位、12点钟位,8个钟点位显著变薄(均P<0.01);与MOG抗体阳性NMOSD组比较,AQP4抗体阳性NMOSD组RNFL层厚度在9点钟位、12点钟位变薄(P<0.05)。结论 AQP4、MOG两种抗体阳性NMOSD患者存在明显的视网膜微观结构的损伤,RNFL层厚度在超过半数象限区域显著变薄;MOG抗体阳性NMOSD较AQP4抗体阳性NMOSD视网膜微观损伤轻。
Objective To investigate the clinical characteristics and retinal microstructure damage of NMOSD patients with AQP4 antibody and MOG antibody. Methods 27 patients with NMOSD were collected from the People s Hospital of Xinjiang Uygur Autonomous Region. At the same time, 20 healthy controls matched by age and sex were selected. The retinal nerve fiber layer was measured by optical coherence tomography(OCT) at 12 clock hours in the NMOSD group and the control group. The retinal nerve fiber layer thickness of AQP4 antibody positive NMOSD and MOG antibody positive NMOSD patients at each clock was compared and analyzed. Results The retinal nerve fiber layer thicknesses of NMOSD patients at 1 o clock( P <0.01), 6 o clock( P =0.036),7 o clock( P =0.025)), 8 o clock( P =0.029), 9 o clock( P <0.01), 10 o clock( P <0.01), 11 o clock( P <0.01) and 12 o clock ( P <0.01)were thinner than the control group;The retinal nerve fiber layer thickness of the AQP4 antibody positive group was thinner than the MOG antibody positive group at 9 o clock and 12 o clock ( P < 0.05). Conclusions Retinal microstructure damage is evident in NMOSD patients, and the retinal nerve fiber layer thickness is significantly thinner in more than half of the quadrants.The retinal microstructure damage of MOG antibody positive NMOSD is less severe than that of AQP4 antibody positive NMOSD.
