摘要
目的:探讨电休克治疗(electroconvulsive therapy,ECT)对抑郁症患者认知功能的短期和长期影响。方法:选验(repeatable battery for the assessment of neuropsychological status,RBANS)对研究组在治疗前(即T1组)、治疗后(即T2组)、治疗结束3个月后(即T3组)三个时间点分别施测,同时评定汉密尔顿抑郁量表(HAMD)、汉密尔顿焦虑(HAMA)量表分值;在与研究组相匹配的治疗前(即C1组)和治疗后(即C2组)的时间点对对照组进行RBNAS测验。结果:抑郁症患者的RBNAS测验总分和5因子得分均低于对照组,且差异有统计学意义(P <0. 05); ECT治疗后患者HAMD、HAMA分值降低,差异有统计学意义(P <0. 05); ECT治疗后言语功能和延时记忆得分低于治疗前,差异有统计学意义(P <0. 05),治疗后3个月,患者的言语功能、注意和延迟记忆得分高于治疗前,差异有统计学意义(P <0. 05);相关分析,RBNAS五组因子得分及总分与教育程度、婚姻(除言语功能外)呈正相关(P <0. 05),与年龄(除言语功能外)呈负相关,与性别无统计学意义(P> 0. 05)。RBNAS总分、注意与病程、HAMD总分呈负相关(P <0. 05),视觉广度与病程呈负相关。结论:ECT治疗可能对抑郁症患者的认知功能有短暂的影响,但对抑郁症患者的认知功能没有产生长期的不良影响,实际上在认知的某些方面甚至有所改善。
Objective: To investigate the short-term and long-term effects of electroconvulsive therapy( ECT) on cognitive function in patients with depression.Methods: Sixty-one hospital patients in Anhui Mental Health Center with depression were recruited as the study group,and 30 norm people were screened as the control group. ECT underwent the repeatable battery for the assessment of neuropsychological status( RBANS) prior to ECT( T1) and at the end of the index( T2). Forty-two patients returned for a 3-month follow-up( T3). All of them were tested by HAMD and HAMA. 30 control subjects were assessed at 2 times for approximately 4 weeks apart( C1 and C2).Results: The scores of RBNAS test and 5 factors in depression patients were significantly lower than those in the control group( P < 0. 05). After treatment of ECT,HAMD and HAMA scores were significantly decreased( P <0. 05). At T2,patients showed deficits in language and delayed memory compared with baseline. At T3,patients showed significant improvement in language,attention and delayed memory compared with baseline( P < 0. 05). Person correlation analysis show that the RBNAS five-factor and total scores were positively correlated with education level and marriage( except for language)( P < 0. 05),negatively correlated with age( except for language),but not compared with sex( P > 0. 05). There was a negative correlation between RBNAS total score,attention and duration,and total score of HAMD( P < 0. 05).Breadth of vision was negatively correlated with disease duration.Conclusion: ECT may have a short-term effect on the cognitive function of depressed patients,but it does not have long-term adverse effects. In fact,it may even improve some aspects of cognitive function in depression.
