摘要
目的探讨超声心动图、心脏声学造影在心肌致密化不全(noncompaction cardiomyopathy, NCCM)诊断中的价值。方法经心脏MRI(cardiac magnetic resonance, CMR)证实NCCM患者22例,确诊前均行超声心动图、心脏声学造影。以CMR为标准,计算超声心动图、心脏声学造影诊断NCCM的准确率,比较3种方法测量的左心室舒张末期容积(left ventricular end-diastolic volume, LVEDV)、左室射血分数(left ventricular ejection fraction, LVEF),及对NCCM节段的检出率。结果超声心动图诊断NCCM的准确率(77.27%)与心脏声学造影(95.45%)比较差异无统计学意义(P>0.05);超声心动图、心脏声学造影和CMR测量的LVEDV[(111.41±18.34)、(114.36±17.99)、(119.68±15.34)mL]、LVEF[(46.45±4.14)%、(43.81±3.99)%、(42.41±3.91)%]比较差异无统计学意义(P>0.05);超声心动图对NCCM心肌节段的检出率(19.25%)低于心脏声学造影(31.28%)和CMR(32.08%)(P<0.05),心脏声学造影与CMR比较差异无统计学意义(P>0.05);心脏声学造影、CMR对心尖段侧壁、后壁、心尖帽及中间段侧壁NCCM心肌节段的检出率高于超声心动图(P<0.05)。结论心脏声学造影对NCCM心肌节段的诊断结果与CMR相当,超声心动图可用于NCCM的初步筛查。
Objective To explore the value of echocardiography and contrast echocardiography to the diagnosis of noncompaction cardiomyopathy(NCCM).Methods Twenty-two patients with NCCM diagnosed by cardiac magnetic resonance (CMR)received echocardiography and contrast echocardiography before diagnosis.The accuracies of echocardiography and contrast echocardiography were calculated by taking CMR result as the standard.The left ventricular end-diastolic volume(LVEDV),left ventricular ejection fraction(LVEF)and detection rates of NCCM by these three methods were compared. Results The accuracy rate showed no significant difference between echocardiography and contrast echocardiography for NCCM (77.27% vs.95.45%)(P>0.05).There were no significant differences in LVEDV ((111.41±18.34),(114.36±17.99),(119.68±15.34)mL)and LVEF((46.45± 4.14)%,(43.81±3.99)%,(42.41±3.91)%)among echocardiography,contrast echocardiography and CMR (P> 0.05).The detection rate of NCCM by echocardiography (19.25%)was significantly lower than that by contrast echocardiography(31.28%)and CMR (32.08%)(P<0.05),and showed no significant difference between contrast echocardiography and CMR(P>0.05).The detection rates of NCCM at the apical lateral wall and posterior wall,apical cap,and lateral wall of middle segment by contrast echocardiography and CMR were significantly higher than those by echocardiography (P<0.05).Conclusion Contrast echocardiography and CMR have similar detection results for NCCM.Echocardiography can provide preliminary screening of NCCM.
