冠心病患者血清脂肪因子与超敏C反应蛋白水平变化及临床意义 被引量:12

Changes and clinical significance of serum Chemerin and hs-CRP level in patients with coronary heart disease

摘要 目的探讨冠心病(coronary heart disease,CHD)患者血清中脂肪因子(Chemerin)与超敏C反应蛋白(high-sensitivity C-reactive protein,hs-CRP)的水平变化及临床意义.方法选取于2016年12月至2018年5月期间我院心内科确诊为CHD且行冠状动脉造影术的患者143例作为研究组,选取同期进行入院进行冠状动脉造影检查单支轻度狭窄,冠状动脉阻塞程度<30%的109例患者为对照组,采集受试者入院次日清晨空腹静脉血检测血清Chemerin、hs-CRP水平,并采用Gensini积分法评估患者冠状动脉病变程度.比较不同病变支数、Gensini积分CHD患者血清Chemerin、hs-CRP水平差异,并分析相关性.结果研究组各亚组血清Chemerin、hs-CRP水平和Gensini积分均高于对照组(P均<0.05).随着CHD患者的病变支数增加,血清Chemerin、hs-CRP水平、Gensini积分均显著增高(P均<0.05);随着Genisini积分增高,血清Chemerin、hs-CRP水平均显著增高,组间差异均有统计学意义(P均<0.05).经Pearson相关性分析,CHD患者血清Chemerin水平与hs-CRP水平呈正相关(r=0.702,P=0.009),血清Chemerin水平、hs-CRP水平均与Genisini积分呈正相关(r=0.643、0.593,P=0.015、0.018).结论血清Chemerin、hs-CRP升高可以反应CHD患者病变累积及病变程度,可以作为反映冠状动脉病变程度的有效指标. Objective To investigate the changes and clinical significance of serum Chemerin and high- sensitivity C-reactive protein (hs-CRP) in patients with coronary heart disease. Methods 143 patients with CHD who underwent coronary angiography in our hospital from December 2016 to May 2018 were selected as the study group. 109 patients who underwent coronary angiography at the same time, with mild stenosis and coronary artery obstruction less than 30% were selected as the control group. Fasting venous blood was collected in the morning after admission and serum Chemerin and hs-CRP level were measured. Gensini integration was used to assess the severity of coronary artery lesions. The serum levels of Chemerin and hs-CRP in patients with CHD with different number of lesion branches and Gensini score were compared and the correlation were analyzed. Results The levels of serum Chemerin and hs-CRP and Gensini score in the study group were higher than those in the control group (all P<0.05). The level of serum Chemerin and hs-CRP and Gensini score increased significantly with the increase of the number of lesion branches in CHD patients (P<0.05). With the increase of Genisini score, the levels of serum Chemerin and hs^CRP increased significantly and the difference between groups was statistically significant (all P<0.05). Pearson correlation analysis showed that serum Chemerin level was positively correlated with hs-CRP level in CHD patients (r=0.702, P=0.009)and serum Chemerin and hs-CRP levels were positively conelated with Genisini score (r=0.643, 0.593, P=0.015, 0.018). Conclusion The elevated levels of serum Chemerin and hs-CRP can reflect the accumulation of lesions and the degree of lesions in CHD patients, that can be used as an effective index to reflect the degree of coronary artery lesions.
