无对比剂肾动脉MRA反转时间在高血压患者中的优化研究

摘要 目的探讨无对比剂肾动脉磁共振血管成像(MRA)反转时间优化在高血压患者中的临床应用。方法选择2017年12月至2018年7月阜外华中心血管病医院收治的高血压患者31例,采用不同的血流抑制反转时间(BSP-TI)对其进行1.5TMR扫描,TI值分别为1100、1200、1300、1400、1500、1600、1700ms。两位观察者评分一致性采用ICC检验,不同组间图像质量主观评分采用单因素方差分析,组间比较采用Bonferroni检验。结果31例高血压患者中,3例患者呼吸频率不能保持相对平稳或频率过快(21次/min以上),予以剔除。两位观察者对28例患者的整体图像主观评分一致性非常好,不同TI值的图像质量组间比较差异有统计学意义(F=42.174,P<0.05),两两组间比较结果显示,当TI值为1400ms时成像质量显示最佳,TI值为<1300ms或>1500ms时,成像质量较差。结论31无对比剂肾动脉MRATI值1400ms时在高血压患者中成像质量显示最佳。 Objective To investigate the clinical application of optimization of inversion time of non-contrast-enhanced renal magnetic resonance angiography(MRA)in hypertensive patients.Methods Thirty-one hypertensive patients admitted to Fuwai Central China Cardiovascular Hospital from December 2017 to July 2018 were selected.1.5T MR scan was performed on all patients with different blood flow suppression inversion time(BSP-TI).Intraclass correlation coefficients(ICCs)was used to evaluate the agreement of the image quality scores of the two readers.The subjective scores of image quality in different groups were assessed by one-way analysis of variance,and Bonferroni test was used for inter-group comparison.Results Among the 31 patients with hypertension,3 cases could not keep their breathing frequency relatively stable or too fast(more than 21 times/min),so they were excluded.The inter reader agreements were excellent for the image quality scores of 28 patients,and there was a significant difference between different TI value groups in image quality(F=42.174,P<0.05).The multiple comparisons showed that when TI=1 400 ms,the image quality was the best,when TI<13 00 ms or>1 500 ms,the image quality was worse.Conclusions When TI=1 400 ms,the image quality of non-contrast-enhanced renal MRA in hypertensive patients is the best.
