摘要
目的探讨高龄女性经辅助生殖技术(ART)受孕后单胎妊娠的围产结局,评估辅助生殖技术对于高龄孕妇的安全性。方法选取2015年6月至2017年6月接受ART治疗获得临床单胎妊娠,并在产科进行孕期保健和分娩的高龄孕妇165例为辅助妊娠组,另选取自然单胎妊娠并且分娩时间相近的高龄孕妇330例为自然妊娠组。采用SPSS 21.0统计软件分析两组孕妇的一般资料及围产结局。结果辅助妊娠组孕妇年龄为(38.6±2.5)岁,自然妊娠组为(36.2±2.9)岁,辅助妊娠组孕妇年龄略高于自然妊娠组(t=2.432,P=0.031)。辅助妊娠组孕妇妊娠期高血压、胎盘粘连比例高于自然妊娠组(P<0.05)。辅助妊娠组孕妇剖宫产率高于自然妊娠组(60.0%比40.3%),差异有统计学意义(P<0.05)。结论辅助妊娠与自然妊娠者多项围产结局未见明显差异,辅助受孕后单胎妊娠对高龄女性是安全的。严格控制辅助生殖技术的应用指征,可减少妊娠期高血压和胎盘粘连等并发症。
Objective To investigate the perinatal outcome of single pregnancy in elderly women after conception of assisted reproductive technology(ART) and to evaluate the safety of assisted reproductive technology for elderly pregnant women.Methods A total of 165 elderly pregnant women who received ART treatment from June 2015 to June 2017 for clinical single pregnancy and health care and childbirth in obstetrics were assisted pregnancy group,and 330 cases of elderly pregnant women with natural single pregnancy and similar delivery time were randomly selected as natural pregnancy group.The general data and perinatal outcome of two groups of pregnant women were analyzed by SPSS 21.0 statistical software.Results The age of pregnant women in the auxiliary pregnancy group was(38.6±2.5),those in the natural pregnancy group was(36.2±2.9) years old,and the age of pregnant women in the auxiliary pregnancy group was slightly higher than that of natural pregnancy group(t=2.432,P=0.031).The proportion of gestational hypertension and placental adhesion in assisted pregnancy group was higher than that in natural pregnancy group(P<0.05).The rate of cesarean section in assisted pregnancy group was higher than that in natural pregnancy group(60% ratio 40.3%),and the difference was significant(P<0.05).Conclusions There is no significant difference in most perinatal outcomes between assisted pregnancy and natural pregnancy,and single pregnancy after assisted conception is safe for elderly women,and strict control of the application of assisted reproductive technology indicated that complications such as hypertension and placental adhesion during pregnancy can be reduced.
