摘要
目的探讨超声心血管整体评分在胎儿心功能不全评估中的应用价值。方法选择在我院接受产前筛查且证实为胎儿心功能不全的80例单胎孕妇与证实胎儿无异常的80例单胎孕妇(2016年1月~2018年1月),设置为观察组与对照组。采用彩色多普勒超声诊断仪对孕妇实施超声检查,评估胎儿水肿情况、脐静脉和静脉导管血流频谱情况、脐动脉血流频谱、心胸比值、心脏瓣膜功能等心血管整体评分指标,比较观察组与对照组胎儿的心血管整体评分,并分析心血管整体评分与胎儿心功能不全的相关性。结果观察组在胎儿水肿情况、脐静脉和静脉导管血流频谱情况、脐动脉血流频谱、心胸比值、心脏瓣膜功能等方面的评分均低于对照组(P <0.05),其心血管整体总分低于对照组(P <0.05),且心血管整体评分与胎儿心功能不全呈负相关。结论采用超声心血管整体评分可对胎儿心功能不全予以反映,可作为胎儿心功能不全的产前辅助诊断方法。
Objective To evaluate the value of ultrasound cardiovascular overall score in evaluating fetal cardiac insufficiency. Methods Eighty cases of singleton pregnant women who received prenatal screening in our hospital and were confirmed to have fetal cardiac insufficiency from January 2016 to January 2018 and 80 cases of singleton pregnant women who were confirmed to have no fetal abnormalities were selected as observation group and control group.The pregnant women were examined by color Doppler ultrasound to evaluate fetal edema,umbilical vein and venous catheter blood flow spectrum,umbilical artery blood flow spectrum,cardiothoracic ratio,cardiac valve function and other cardiovascular overall score indicators.The total cardiovascular score of the observation group and the control group was compared,and the correlation between the total cardiovascular score and fetal cardiac insufficiency was analyzed. Results The scores of fetal edema,umbilical vein and venous catheter blood flow spectrum,umbilical artery blood flow spectrum,cardiothoracic ratio and heart valve function in the observation group were all lower than those in the control group(P < 0.05).The total cardiovascular score was lower than the control group(P < 0.05),and the total cardiovascular score was negatively correlated with fetal cardiac insufficiency. Conclusion Ultrasonic cardiovascular which reflected the overall score of fetal cardiac insufficiency,can be used as the prenatal auxiliary diagnosis of fetal cardiac insufficiency.
