超声在子痫前期胎儿心功能评价中的应用效果研究 被引量:1

摘要 目的探究超声在子痫前期胎儿心功能评价中的应用效果。方法整群选择在该院进行产前检查并分娩的48例子痫前期孕妇作为观察组,并选择同期正常分娩的48例孕妇作为对照组。对比两组胎儿的心脏收缩功能、舒张功能及左室Tei指数。结果观察组的EF、FS明显低于对照组(P<0.05),而两组胎儿的AO-PSV、MVE、MVA及E/A比值比较差异无统计学意义(P>0.05)。而观察组的ICT+IRT、ET以及Tei指数分别均显著高于对照组(P<0.05)。结论子痫前期胎儿的收缩功能有明显变化,通过超声检查能够有效对子痫前期胎儿的心功能进行评价,而Tei指数在评价子痫前期胎儿的心功能上较EF、E/A比值更加敏感。 Objective To discuss the application effect of ultrasound in the evaluation of fetal heart function in preeclampsia. Methods 48 cases of pregnant women in preeclampsia receiving prenatal examination and giving birth in our hospital from February 2015 to November 2015 were selected as the observation group, 48 cases of normal pregnant women giving birth in our hospital at the same period were selected as the control group, the cardiac systolic function, diastolic function and left ventricular Tei index of the observation group were detected by ultrasonic Doppler, and compared with various indexes of the control group. Results The ejection fraction and fractional shortening in the observation group were obviously lower than those in the control group, [(68.23±3.13%,33.63±1.85%) vs(70.21±2.86%,35.18±2.43%)] and the differences were statistically significant P〈0.05, there was no obvious difference in the aorta blood flow velocity peak between the two groups PP〉0.05, the differences in the MVE(?mitral valve orifice peak E blood flow velocity), MVA(mitral valve orifice peak A blood flow velocity) and E/A ratio between the two groups were not statistically significant P〉0.05, the left ventricular isometric time, left ventricular ejection time and Tei index in the observation group were obviously higher than those in the control group, [(81.23±6.68)ms,(171.35±9.230)ms,(0.408±0.026) vs(68.81±6.53) ms,(175.68±10.68) ms,(0.386±0.031)],and the differences were statistically significant, P〈0.05. Conclusion The systolic function of fetuses in preeclampsia has an obvious change, and the ultrasonic examination can effectively evaluate the heart function of fetuses in preeclampsia, Tei index is more sensitive in evaluating the heart function of fetuses in preeclampsia compared with ejection fraction and E/A ratio.
