摘要
目的探讨母牛分枝杆菌辅助治疗肺结核合并糖尿病疗效及对患者糖化血红蛋白与T淋巴细胞亚群的影响。方法收集我院2016年3月~2018年4月收治的102例肺结核合并糖尿病患者作为观察对象,按随机数字表法分为观察组与对照组,各51例。对照组采取常规抗结核方案联合胰岛素治疗,观察组则增加母牛分枝杆菌辅助治疗,比较两组痰菌阴转率、病灶转归及空洞转归情况,检测两组治疗前后糖化血红蛋白与T淋巴细胞亚群变化。结果观察组痰菌阴转率、病灶吸收率及空洞闭合率均明显高于对照组(P<0.05);治疗后,观察组HbA1c水平明显低于对照组(P<0.05);治疗后,观察组CD4^+、CD4^+/CD8^+水平明显高于对照组,CD8^+水平明显低于对照组(P<0.05);两组不良反应总发生率比较,差异无统计学意义(P>0.05)。结论采用母牛分枝杆菌辅助治疗能够有效下调肺结核合并糖尿病患者血糖水平,调节T淋巴细胞亚群,改善机体免疫功能。
Objective To investigate the effect of Mycobacterium vaccae adjuvant therapy on pulmonary tuberculosis with diabetes mellitus and its effect on glycosylated hemoglobin and T lymphocyte subsets in patients. Methods A total of 102 patients with pulmonary tuberculosis and diabetes mellitus admitted in our hospital from March 2016 to April 2018 were enrolled as observation subjects. The patients were divided into observation group and the control group according to the random number table method, with 51 cases in each group. The control group received conventional anti-tuberculosis regimen combined with insulin therapy, and the observation group was also treated with the adjuvant treatment of M.vaccae. The sputum negative conversion rate, lesion outcome and cavity outcome were compared between the two groups. Changes in glycated hemoglobin and T lymphocyte subsets were detected before and after treatment. Results The incidence of sputum negative rate, lesion absorption rate and cavity closure rate in the observation group were significantly higher than those in the control group (P<0.05). After treatment, the HbA1c level in the observation group was significantly lower than that in the control group(P<0.05). After treatment, the levels of CD4^+ and CD4^+/ CD8^+ in the observation group were significantly higher than those in the control group, and the level of CD8^+ was significantly lower than that in the control group(P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05). Conclusion The adjuvant treatment with M.vaccae can effectively lower the blood glucose level of pulmonary tuberculosis patients with diabetes, regulate the T lymphocyte subsets, and improve the immune function of the body.
