不同屏气状态下宽探测器智能毫安控制第五代迭代重建技术在冠状动脉成像中的应用研究 被引量:1

An investigation on the application of wide-body CT detector with intelligent miliiampere control and fifth generation iterative reconstruction in coronary CTA with/without breath holding

摘要 目的探讨不同屏气状态下Revolution CT的宽体探测器、超快速扫描、智能毫安(mAs)控制、第五代迭代重建技术在冠状动脉(CCTA)应用中的图像质量优良率。方法符合纳入标准的CCTA受检者共200例,男111例,女89例,年龄34~86岁,平均(60.37±11.05)岁。随机分为不屏气组和屏气组各100例。依据Likert分级法主观评价图像质量;测量左、右冠状动脉开口、升主动脉根部的CT值,计算影像的信噪比(SNR)、对比噪声比(CNR)来客观评价图像质量。同时记录受检者的CT剂量指数、剂量长度乘积(DLP)、对比剂注射的容量和浓度,分别计算受检者辐射剂量和碘用量,并进行统计学分析。结果两组患者年龄、心率、体重、身高、体重指数的差异均无统计学意义(P> 0.05)。两组的CT值、SNR及CNR的差异均有统计学意义(P <0.05)。两组右冠状动脉血管优良率差异无统计学意义(P> 0.05);两组左冠状动脉、回旋支血管优良率差异均有统计学意义(P <0.05);两组300支血管图像优良率分别为92%、98.67%,χ~2=14.985,P=0.000。不屏气组对比剂总量、碘总量稍高于屏气组,差异均有统计学意义(P <0.05)。不屏气组长度、DLP、ED明显低于屏气组,差异均有统计学意义(P <0.05)。两组mAs差异无统计学意义(P> 0.05)。结论不同呼吸状态下Revolution CT的宽体探测器、智能毫安控制、快速扫描、第五代超低剂量迭代重建等技术在冠脉CCTA上应用安全无创、便捷可行,图像优良,推荐屏气扫描。 Objective To investigatethe image quality of Revolution CT usingwide-body detector with ultra-fast scanning,intelligent miliiampere control,fifth generation iterative reconstruction with and without breath holding states in coronary CTA.Methods 200 cases(111 males,89 females,age 34~86 years,mean age 60.37±11.05 years)were randomly equally divided into 2 groups of 1(breathless),2(breath).The quality of images was evaluated subjectively based on Likert grading method.The CT values at the entrance of left and right coronary arteries and the ascending aortic root were measured.The signal-to-noise ratio(SNR)and contrast-to-noise(CNR)of the images were calculated to evaluate the image quality objectively.Simultaneously the CT dose index,dose length product(DLP),dose and concentration of injected contrast were recorded.And the radiation dose and iodine dose were calculated respectively,and statistically analyzes was performed.Results There was no significant difference in age,heart rate,body weight,height,BMI between the 2 groups(P>0.05).Significant differences were found respectively in CT value,SNR and CNR between the 2 groups(P<0.05).And no significant difference between 2 groups in showing vessels good degree of RCA,while significant differences in LAD,LCX with total degree 92.00﹪,98.67﹪respectively(P=0.000).There were respectively significant differences in contrast dose,iodine dose in group 2 higher than those in group 1(P<0.05),and the same in L,DLP and ED in group 2 lower than those in group 1 respectively(P<0.05),yet no difference in mAs.Conclusions Wide-body CT detector,intelligent milliampere control,ultra-fastscanning,the fifth generation of ultralow dose iterative reconstruction technology in different breath holding states in application of coronary artery CTA is safe,noninvasive,convenient and feasible with excellent images.And breathless scanning is recommended.
