冠状动脉CT成像的个体化低对比剂剂量优化研究 被引量:14

摘要 目的探讨128层双源Flash CT大螺距冠状动脉成像低对比剂剂量优化时的可行性。方法将61例行冠状动脉CTA检查的患者随机分为两组(A组30例,B组31例),按照对比剂剂量和体质量的比值分别为0.7ml/kg和0.5 ml/kg,各组的对比剂剂量等于体质量乘以各自的比值。测量肺动脉、主动脉、左心房、左心室、冠状动脉各分支血管以及心肌下壁的CT值。采用独立样本t检验评价两组血管、心腔及心肌下壁CT值、对比剂剂量和有效辐射剂量,采用χ~2检验比较两组间冠状动脉图像质量分级显示段数。结果两组中各冠状动脉主干及其分支血管清晰显示,对比剂剂量分别为(47±8)ml、(36±6)ml,差异有统计学意义(t=5.343,P=0.020)。升主动脉CT值分别为(454±61)HU、(439±77)HU,差异无统计学意义(t=-0.794,P=0.430)。A、B两组冠状动脉分别有374与386段,评分为Ⅰ级与Ⅱ级的段数分别有368段和378段,所占总冠状动脉段数的百分比分别为98.4%和97.9%,差异无统计学意义(χ~2=0.230,P=0.631)。辐射剂量分别为(0.88±0.04)mS v、(0.85±0.07)mS v,差异无统计学意义(t=2.087,P=0.087)。结论 128层双源CT冠状动脉成像在采用对比剂剂量和体质量之比为0.5ml/kg时,既可以满足临床诊断,又可以最大程度的降低对比剂剂量。 Objective To evaluate the feasibility of high-pitch Coronary Computed Tomography Angiography with Low Dose Contrast Medium optimization in 128-slice Dual-source CT. Methods 61 patients who underwent coronary CTA were randomly divided into two groups,Group a,30 cases,Group B,40 cases. The contrast medium volume per patient in each group was equal to body mass multiplied by the ratio of each group. The ratio of group A is 0. 7ml / kg and group B is 0. 5ml / kg. The contrast medium volume,the radiation dose and the CT value of Superior vena cava,pulmonary artery,aorta,left atrium,left ventricle,various coronary branches of blood vessels and myocardium under the walls were compared using two sample t-test. The numbers corresponding to the coronary artery image quality grading were compared by χ~2-test.Results The coronary artery trunk and branches of two groups were displayed clearly. The contrast medium volume of group B was lower than group A( t = 5. 343,P = 0. 020) 。No difference of the aortal CT value between the two groups( t=- 0. 794、P = 0. 430). No statistical difference in radiation dose between the two groups( t = 2. 087,P = 0. 087). No statistical difference in the proportion of excellent and good numbers to all coronary numbers between the two groups( χ~2=0. 230,P = 0. 631). Conclusion High-pitch Coronary CTA in 128-slice Dual-source CT with the contrast medium volume,which equals the body mass multiplied by the ratio 0. 5ml / kg,can minimize the amount of contrast agent greatly while meeting the clinical requirements.
