摘要
目的回顾分析胰腺实性假乳头状瘤(solid pseudopapillary tumor of pancreas, SPTP)的CT表现,总结其不典型征象。方法回顾性分析18例手术病理证实的SPTP病例的临床及术前CT资料,分析患者的性别、年龄,分析SPTP的部位、形态、边缘、内部密度、强化方式及周围结构,总结不典型表现,比较男女患者SPTP的CT表现的差异。结果出现的不典型征象包括:发生于胰腺外(1例)、实性(4例)或囊性(2例)成分为主、分叶状(4例)、胰管扩张(3例)、胆管扩张(1例)、胰腺实质萎缩(1例)、侵犯周围结构(3例)、肝脏转移(1例)。与女性患者相比,男性患者(5例)平均年龄较大,其肿瘤完整包膜出现概率较低,实性为主型及分叶状所占比例较大。结论 SPTP的CT表现可不典型,需要加深对不典型表现的认识。
Objective To discuss the atypical CT manifestations of solid pseudopapillary tumors of the pancreas (SPTP). Methods The clinical and CT data of 18 patients with pathologically confirmed SPTP were retrospectively reviewed. The proportion of atypical cases was calculated. Atypical CT manifestations were analyzed and summarized. The features of SPTP were compared between male and female patients. Results Atypical manifestations included ectopic SPTP (one case), SPTP with mostly solid component (four cases) or with mostly cystic component (two cases), lobulated shape (four cases), pancreatic duct dilatation (three cases), bile duct dilatation (one case), atrophy of the pancreas (one case), invasion of the surrounding structure (three cases), and liver metastasis (one case). Male patients (five cases) tended to be older. SPTP in male patients appeared as mainly solid mass with lobulated contour and no capsule. Conclusion SPTP shows diverse CT manifestations. A fully understanding of the atypical appearance may help to reduce misdiagnosis.
